中西医结合护理联合呼吸训练对老年慢性心力衰竭患者生活质量及心功能的影响

摘　　要：目的 :探讨中西医结合护理联合呼吸训练对老年慢性心力衰竭(chronic heart failure,CHF)患者生活质量及心功能的影响。方法:将86例CHF患者随机分为观察组与对照组各43例,两组均进行常规内科药物治疗,对照组同时采取常规护理,观察组在对照组基础上加中西医结合护理联合呼吸训练,每个疗程30天,共3个疗程。分别于治疗前后进行中医证候评分、生活质量评分(quality of life,QOL)及6 min步行距离试验(six minute walk distance,6MWD)、心功能指标[左心室射血分数(left ventricular ejection fraction,LVEF)、左室舒张末内径(left ventricular end-diastolic dimension,LVEDD)、左心室舒张末期容量(left ventricular end diastolic volume,LVEDV)、左心室收缩末期容量(left ventricular end systolic volume,LVESV)]心衰标志物水平[B型尿钠肽(B-type brain natriuretic peptide,BNP)、血清激活素A(activin A,ACT-A)]、检测,并观察患者护理满意度。结果:与治疗前相比,两组患者中医证候评分、LVEDD、LVEDV、LVESV、BNP、ACT-A均降低(P<0.05),QOL评分、6MWD、LVEF均升高(P<0.05),观察组改善更明显(P<0.05);观察组患者护理总满意率高于对照组(P<0.05)。结论:中西医结合护理联合呼吸训练可以有效改善老年CHF患者临床症状与生活质量,提高患者运动功能,恢复心脏功能。Objective:To discuss the influence of integrated traditional Chinese and Western medicine nursing combined with respiratory training on quality of life and heart function of elderly patients with chronic heart failure(CHF).Methods:All 86 CHF patients were randomized into the observation group and the control group,with 43 cases in each group,both groups were treated by routine internal medicine,the control group adopted conventional care,and the observation group integrated traditional Chinese and Western medicine nursing combined with respiratory training based on the therapy the control group adopted,the course of treatment was 30 days,for three courses of the treatment.To detect TCM syndrome integrals,QOL scores,6MWD,the indexes of heart function(LVEF,LVEDD,LVEDV and LVESV)and the levels of heart failure markers(BNP and ACT-A)respectively before and after the treatment,and observe the patients’nursing satisfaction degrees.Results:Compared with before the treatment,TCM syndrome integrals,LVEDD,LVEDV,LVESV,BNP and ACT-A reduced in the two groups(P<0.05),QOL scores,6MWD and LVEF raised(P<0.05),the improvements of the observation group were more noticeable(P<0.05);the observation group was higher than the control group in the patients’nursing satisfaction degrees(P<0.05).Conclusion:Integrated traditional Chinese and Western medicine nursing combined with respiratory training could effectively improve clinical symptoms and quality of life of elderly patients with CHF,raise the patients’motor function and restore heart function.
