摘　　要：目的研究在宫颈长度(CL)<25 mm时,宫颈角(UCA)是否与早产的风险相关。方法选取2021年5月1日至2022年5月1日在深圳市龙华区人民医院行超声检查,妊娠18~23^(+6)周的孕妇180例,一共分为6组:a组18~18^(+6)周,b组19~19^(+6)周,c组20~20^(+6)周,d组21~21^(+6)周,e组22~22^(+6)周,f组23~23^(+6)周,每组各30例。通过超声测量UCA和CL,CL<25 mm时,根据分娩的孕周分为足月组和早产组,评估UCA对早产预测的效能。结果早产组平均UCA为(124.6±3.7)°,大于足月组的(93.8±1.7)°,差异有统计学意义(t=8.433,P=0.026)。UCA对早产预测的效能:UCA在118°的敏感度为59.5%,特异度为91.8%,ROC曲线下面积0.826。结论UCA是一种有效预测早产的指标,当UCA≥118°时,应高度警惕早产的发生。Objective To investigate whether the uterocervical angle(UCA)is associated with preterm birth when the cervical length(CL)is less than 25 mm.Methods A total of 180 pregnant women with 18-23^(+6)weeks of gestation who underwent ultrasound examination at the People’s Hospital of Longhua Shenzhen from May 1,2021 to May 1,2022 were selected and divided into 6 groups:18-18^(+6)weeks in Group A,19-19^(+6)weeks in Group B,20-20^(+6)weeks in Group C,21-21^(+6)weeks in Group D,22-22^(+6)weeks in Group E,and 23-23^(+6)weeks in Group F,with 30 cases in each group.These women were also divided into a full-term group and a preterm group according to the gestational week upon delivery based on ultrasound measurements of UCA and CL(less than 25 mm),and the efficiency of UCA in predicting preterm birth was evaluated.Results The average UCA of the preterm group was(124.6±3.7)°,which was higher than that of the full-term group(93.8±1.7)°,with statistically significant difference(t=8.433,P=0.026).The efficiency of the UCA in predicting preterm birth:the sensitivity was 59.5%,the specificity was 91.8%,and the area under the receiver operating characteristic(ROC)curve was 0.826 when the UCA was at 118°.Conclusion The UCA is a valid predictor of preterm birth,and a high degree of vigilance should be exercised when the UCA is≥118°.
