复方黄柏液涂剂联合负压封闭引流治疗藏毛窦皮瓣转移术后感染的临床效果分析

摘　　要：目的探讨复方黄柏液涂剂联合负压封闭引流(VSD)治疗藏毛窦皮瓣转移术后感染的应用效果。方法收集2013年1月至2022年1月中国中医科学院西苑医院收治的藏毛窦皮瓣转移术后感染后利用VSD联合复方黄柏液涂剂进行冲洗治疗患者的临床资料,记录患者术后因感染引发的并发症类型,以及治疗后21 d的24 h疼痛评分、愈合时间、二次并发症发生和复发率,评价其治疗效果。结果共收集19例患者,采用Limberg、Karydakis和BascomⅡ3种手术方法治疗,随访6~96个月,平均(19.5±8.2)个月;Limberg手术患者的疼痛反应较其他2种手术重,1例Limberg手术患者因非吸收线线结异物而导致感染复发,Karydakis和BascomⅡ手术患者均正常愈合,未出现复发。结论复方黄柏液涂剂联合VSD治疗藏毛窦皮瓣转移术后感染切实有效。Objective To explore the clinical effect of compound huangbai liquid coating combined with vacuum sealing drainage(VSD)in the treatment of infection after transfer of pilonidal sinus flap.Methods Clinical data of patients treated with Huangbai liquid coating combined with VSD for irrigation after the infection of the skin flap transferred from Xiyuan Hospital,China Academy of Chinese Medical Sciences from January 2013 to January 2022 were collected.The types of postoperative complications caused by infection were recorded,and the 24 hours pain scores,healing time,secondary complications and recurrence rate after 21 days of treatment were compared to evaluate the therapeutic effect.Results A total of 19 patients were collected and treated with Limberg,Karydakis and BascomII.The patients were followed up for 6 to 96 months,with an average of(19.5±8.2)months.The pain reaction in the Limberg surgery was more severe than that in the other two types of surgery.One patient underwent Limberg surgery had a recurrence of infection caused by non-absorbed suture knot foreign bodies.Both Karydakis and BascomII surgery patients healed normally without recurrence.Conclusion Compound huangbai liquid coating combined with VSD is effective in the treatment of infection after pilonary sinus flap transfer.
