银星养脑片治疗弥漫性轴索损伤瘀阻脑络证的临床研究

摘　　要：目的:探讨银星养脑片治疗弥漫性轴索损伤(Diffuse Axonal Injury, DAI)瘀阻脑络证的临床疗效。方法:采用随机数字表法将100例DAI患者随机分为对照组和观察组各50例。对照组给予DAI患者常规西医治疗,观察组在对照组的基础上加用银星养脑片,疗程均为2 w。检测治疗前后脑电双频指数(Bispectral Index, BIS)、DAI生物标志物、炎症因子,并评价格拉斯哥评分(GCS评分)和临床疗效。结果:观察组总有效率为70%,优于对照组的58%(P<0.05)。与治疗前比较,治疗后对照组和观察组的BIS值、GCS评分均明显增高(P<0.05);DAI生物标志物(NSE、S100B)、炎症因子(IL-6、TNF-α)含量均明显降低(P<0.05);治疗后与对照组比较,观察组BIS值、GCS评分均明显增高(P<0.05),DAI生物标志物(NSE、S100B)、炎症因子(IL-6、TNF-α)含量均明显降低(P<0.05),观察期间两组均无不良反应发生。结论:银星养脑片治疗DAI能修复患者受损神经、保护脑组织、提高生存质量、降低致残率,其机制可能与修复轴膜通透性、纠正离子通道异常、拮抗中枢神经炎症反应有关。Objective:To explore the clinical efficacy of Yinxing Yangnao(银星养脑)Tablets in the treatment of diffuse axonal injury(DAI)with Yuzunaoluo(瘀阻脑络)Syndrome.Methods:A total of 100 patients with DAI were randomly divided into a control group(n=50)and an observation group(n=50)according to a random number table.All patients were treated with DAI-targeted conventional western medicine,while those in the observation group received additional Yinxing Yangnao Tablets.The course of treatment was two weeks.Bispectral index(BIS),Glasgow Coma Scale(GCS)score,DAI biomarkers,and inflammatory factors were measured before and after treatment,and the clinical efficacy was evaluated.Results:The total effective rate of the observation group was 70.00%,higher than 58.00%of the control group(P<0.05).After treatment,the BIS and CCS scores of the two groups were higher than those before treatment(P<0.05),and the DAI biomarkers(NSE and S100B)and inflammatory factors(IL-6 and TNF-α)were lower than those before treatment(P<0.05).After treatment,the BIS and GCS scores of the observation group were higher than those of the control group(P<O.05),and the DAI biomarkers(NSE and S100B)and inflammatory factors(IL-6 and TNF-α)were lower than those of the control group(P<0.05).There were no adverse reactions in the two groups during the observation period.Conclusion:Yinxing Yangnao Tablets can repair damaged nerves,protect brain tissues,improve quality of life,and reduce disability rate in the treatment of DAI.The underlying mechanism is presumably related to repairing axial membrane permeability,correcting ion channel abnormalities,and antagonizing central nervous inflammatory response.
