筋骨痛消丸治疗膝骨关节炎的药物经济学评价

摘　　要：目的评价筋骨痛消丸对比骨刺片、双氯芬酸钠缓释片治疗膝骨关节炎的成本-效果。方法基于卫生系统视角,根据疾病治疗转归情况建立决策树模型,采用文献综述及单组率Meta方法获得模型参数,对筋骨痛消丸、骨刺片及双氯芬酸钠缓释片治疗方案进行成本-效果分析,并对参数的不确定性进行敏感性分析。结果筋骨痛消丸的治疗有效率高于骨刺片(91.70%vs 86.80%),增量成本-效果比(ICER)为3.46元,当每个有效率百分点的意愿支付(WTP)值大于3.40元时,筋骨痛消丸组经济性更优。筋骨痛消丸有效率也高于双氯芬酸钠缓释片(91.70%vs82.70%),ICER为11.55元,当WTP值大于11.15元时,筋骨痛消丸组经济性更优。结论筋骨痛消丸对比骨刺片和双氯芬酸钠缓释片治疗膝骨关节炎的效果更优而成本相对较高,在满足一定WTP水平的前提下该药物临床应用具有经济性。Objective To evaluate the cost-effectiveness of Jingutongxiao pill,Guci tablet and diclofenac sodium sustained-release tablet on the treatment of knee osteoarthritis.Methods Based on healthcare system perspective,a decision tree model was applied with parameters obtained from literature review and meta analysis to analyze the cost-effectiveness of Jingutongxiao pill,Guci tablet and diclofenac sodium sustained-release tablet.One-way sensitivity analysis and probabilistic sensitivity analysis were conducted.Results Compared with Guci tablet,Jingutongxiao pill had better effect(91.70%vs 86.80%)and got an incremental cost-effectiveness ratio of 3.46 yuan.When the willingness to pay was higher than 3.40 yuan,Jingutongxiao pills was cost-effectivness.Compared with diclofenac sodium sustained-release tablet,Jingutongxiao pill had better effect(91.70%vs 82.70%)and got an incremental cost-effectiveness ratio of 11.55 yuan.Jingutongxiao pills was cost-effectivness when willingness to pay was higher than 11.15 yuan.Conclusion Jingutongxiao pill had better effect and higher cost on the treatment of knee osteoarthritis compared with Guci tablet and diclofenac sodium sustained-release tablet.The clinical application of Jingutongxiao pill was cost-effectivness when suitable willingness to pay figure was set.
