摘 要:目的探讨热性惊厥患儿血清微小RNA-148a-3p(miR-148a-3p)、高迁移率族蛋白B1(HMGB1)、25-羟维生素D(25(OH)D)水平与癫痫发作的相关性。方法回顾性分析2018-01—2022-03南阳市中心医院收治的122例热性惊厥患儿,根据热性惊厥患儿预后结局是否发生癫痫,分为热性惊厥癫痫组36例和热性惊厥非癫痫组86例,同期选择55例未发生惊厥的呼吸道感染仅发热的患儿为对照组,比较3组患儿的血清miR-148a-3p、HMGB1、25(OH)D水平。收集3组患儿临床资料进行多因素Logistic回归分析,绘制ROC曲线分析miR-148a-3p、HMGB1、25(OH)D对热性惊厥患儿癫痫发作的预测价值。结果热性惊厥癫痫组的血清miR-148a-3p表达量、HMGB1水平高于热性惊厥非癫痫组、对照组[(2.26±0.57)vs(1.71±0.43)vs(1.50±0.38);(7.45±1.87)μg/L vs(6.70±1.58)μg/L vs(5.33±1.29)μg/L;均P<0.05],血清25(OH)D水平低于热性惊厥非癫痫组、对照组[(26.83±6.79)μg/L vs(34.24±8.56)μg/L vs(42.16±4.15)μg/L;P<0.05]。惊厥类型、惊厥持续时间、退热后脑电图结果、miR-148a-3p、HMGB1、25(OH)D与癫痫发作存在相关关系(P<0.05)。miR-148a-3p、HMGB1、25(OH)D预测热性惊厥患儿癫痫发作的灵敏度为83.33%、77.78%、72.22%,特异度为74.42%、68.60%、63.95%。结论血清miR-148a-3p、HMGB1、25(OH)D水平与热性惊厥患儿癫痫发作有一定相关性,可作为临床辅助诊断标志物。Objective To investigate the relationship between serum microRNA-148a-3p(miR-148a-3p),high mobility group protein B1(HMGB1)and 25-hydroxyvitamin D(25(OH)D)levels and epileptic seizure in children with febrile convulsion.Methods This study retrospectively reviewed 122 children with febrile convul⁃sion who were admitted to the Nanyang Central Hospital from January 2018 to March 2022.According to the presence or absence of epilepsy,the children were divided into epilepsy group(36 cases)and non-epilepsy group(86 cases).At the same time,55 children with respiratory tract infection and fever but without convulsions were selected as the control group.Serum miR-148a-3p,HMGB1 and 25(OH)D levels in the 3 groups were comparatively analyzed.The clinical data were collected for multivariate logistic regression analysis,and ROC curve was plotted to analyze the predictive value of miR-148a-3p,HMGB1 and 25(OH)D for epileptic seizures in children with febrile convulsion.Results The expression level of miR-148a-3p and HMGB1 level in the epilepsy group were higher than those in the non-epilepsy group and the control group[(2.26±0.57)vs(1.71±0.43)vs(1.50±0.38);(7.45±1.87)μg/L vs(6.70±1.58)μg/L vs(5.33±1.29)μg/L;P<0.05],and serum 25(OH)D level was lower than that in the non-epilepsy group group and the control group[(26.83±6.79)μg/L vs(34.24±8.56)μg/L vs(42.16±4.15)μg/L;P<0.05].The type and duration of convulsion,EEG results after fever abatement,miR-148a-3p,HMGB1,and 25(OH)D were correlated with epileptic seizures(P<0.05).The sensitivity of miR-148a-3p,HMGB1,and 25(OH)D to predict epileptic seizures in children with febrile convulsion was 83.33%,77.78%,and 72.22%,respectively.The specificity was 74.42%,68.60%,and 63.95%,respectively.Conclusion Serum miR-148a-3p,HMGB1,and 25(OH)D levels are related to epileptic seizure in children with febrile convulsion,which indicates that these indicators can be used as clinical auxiliary diagnostic markers.
