摘  要:目的探讨选择性亚低温联合奥拉西坦在足月新生儿缺氧缺血性脑病(HIE)中的应用效果及对患儿生长发育的影响。方法选取2020年3月至2022年3月我院收治的50例足月HIE患儿,随机分为观察组与对照组。对照组采用选择性亚低温治疗,观察组在对照组基础上采用奥拉西坦治疗。比较两组的神经行为、智力发育及生化指标。结果观察组出生后7 d、14 d、28 d的NBNA评分高于对照组(P<0.05)。观察组3月龄、6月龄的MDI、PDI评分均高于对照组(P<0.05)。治疗后,两组的HMGB1、HIF-1α水平降低,IGF-1水平升高,且观察组HMGB1、HIF-1α水平低于对照组,IGF-1水平高于对照组(P<0.05)。结论选择性亚低温联合奥拉西坦可改善足月HIE患儿神经行为及智力发育,远期疗效较好,可能与调节HMGB1、HIF-1α、IGF-1表达有关。Objective To explore the application effect of selective mild hypothermia combined with oxiracetam in full-term neonatal hypoxic-ischemic encephalopathy(HIE)and the impact on growth and development of children.Methods 50 full-term HIE children admitted to our hospital from March 2020 to March 2022 were selected and randomly divided into observation group and control group.The control group was treated with selective mild hypothermia,and the observation group was treated with oxiracetam on the basis of the control group.The neurobehavior,intellectual development and biochemical indicators were compared between the two groups.Results The NBNA scores at 7 days,14 days and 28 days after birth of the observation group were higher than those of the control group(P<0.05).The MDI and PDI scores at 3 months old and 6 months old of the observation group were higher than those of the control group(P<0.05).After treatment,the HMGB1 and HIF-1αlevels of the two groups decreased,and the IGF-1 levels increased(P<0.05);The HMGB1 and HIF-1αlevels of the observation group were lower than those of the control group,and the IGF-1 level was higher than that of the control group(P<0.05).Conclusions Selective mild hypothermia combined with oxiracetam can improve the neurological function and intellectual development of full-term HIE children,with better long-term effect,which may be related to the regulation of HMGB1,HIF-1αand IGF-1 expressions.
