经皮穿刺椎体成形术治疗老年OVCF的最佳手术时机探讨

摘　　要：目的探讨经皮穿刺椎体成形术(PKP)治疗老年骨质疏松性椎体压缩骨折(OVCF)的最佳手术时机。方法选取2019年6月至2022年6月本院收治的150例老年OVCF患者,按照手术时机分为A组(骨折7 d内手术)、B组(骨折7~28 d内手术)、C组(骨折28 d后手术)。比较三组的治疗效果及并发症。结果A组、B组骨水泥渗漏率低于C组(P<0.05)。三组的再骨折率比较差异无统计学意义(P>0.05)。A组、B组Cobb角低于术前及C组(P<0.05)。A组、B组Owestry功能障碍指数(ODI)评分低于C组(P<0.05)。术后6个月,三组矢状面偏移(SVA)、胸椎后凸角(TK)升高,腰椎前凸角(LL)降低,C组LL低于A组、B组(P<0.05)。结论PKP治疗老年OVCF的效果确切,骨折后28 d内接受治疗患者术后骨水泥渗漏发生率低,且利于患者功能恢复。Objective To explore the best operation timing for percutaneous vertebroplasty(PKP)in the treatment of elderly osteoporotic vertebral compression fracture(OVCF).Methods 150 elderly OVCF patients admitted to our hospital from June 2019 to June 2022 were divided into group A(operation within 7 days of fracture),group B(operation within 7 to 28 days of fracture)and group C(operation after 28 days of fracture)according to operation time.The treatment effect and complications of the three groups were compared.Results The leakage rates of bone cement in group A and group B were lower than that in group C(P<0.05).No statistical difference was found in the refracture rate among the three groups(P>0.05).The Cobb angle in group A and group B were lower than that before operation and that in group C(P<0.05).The ODI scores of group A and group B were lower than that of group C(P<0.05).6 months after operation,the sagittal vertical axis(SVA)and thoracic kyphosis(TK)increased,lumbarlordo-sis(LL)decreased in three groups,and the LL in group C was lower than those in group A and group B(P<0.05).Conclusions PKP has exact effect in the treatment of elderly OVCF.The treatment within 28 days after fracture has low incidence of bone cement leakage in patients,which is beneficial to the functional recovery.
