慢性乙型病毒性肝炎产妇乙肝病毒载量与母乳喂养后婴儿乙肝标志物的相关性研究

摘　　要：目的研究慢性乙型病毒性肝炎(CHB)产妇乙肝病毒载量与母乳喂养后婴儿乙肝标志物的相关性。方法采集我院收治的91例CHB产妇血清进行乙型肝炎病毒(HBV)-DNA检测,并检测喂养前后婴儿乙肝标志物水平,分析CHB产妇乙肝病毒载量与母乳喂养后婴儿乙肝标志物的相关性。结果>10^(3)IU/mL组产妇感染状态严重于≤10^(3)IU/mL组(P<0.05)。>10^(3)IU/mL组婴儿喂养后的HBsAb、HBsAg、HBeAg、HBeAb、HBcAb水平高于≤10^(3)IU/mL组(P<0.05)。产妇血清HBV-DNA表达与婴儿喂养后的HBsAb、HBsAg、HBeAg、HBeAb、HBcAb水平呈正相关(P<0.05)。结论CHB产妇乙肝病毒载量越高则喂养后婴儿乙肝标志物水平越高,临床需充分重视,依据HBV-DNA表达情况指导喂养方式,以降低HBV感染母婴传播风险。Objective To explore the correlation between hepatitis B virus load of chronic hepatitis B(CHB)puerperae and hepatitis B markers of infants after breastfeeding.Methods The serum of 91 CHB puerperae admitted to our hospital were collected for hepatitis B virus(HBV)-DNA detection,the levels of hepatitis B markers of infants before and after breastfeeding were detected,and the correlation between hepatitis B viral load of CHB puerperae and hepatitis B markers of infants after breastfeeding was analyzed.Results The maternal infection status in the>10^(3)IU/mL group was more serious than that in the≤10^(3)IU/mL group(P<0.05).The HBsAg,HBeAg,HBeAb and HBcAb levels of infants after breastfeeding in the>10^(3)IU/mL group were higher than those in the≤10^(3)IU/mL group(P<0.05).The maternal serum HBV-DNA expression was positively correlated with the HBsAb,HBsAg,HBeAg,HBeAb and HBcAb levels of infants after breastfeeding(P<0.05).Conclusions The higher the hepatitis B viral load of CHB puerperae,the higher the hepatitis B marker levels of infants after breastfeeding.It is necessary to pay full attention to the clinical practice and guide feeding methods according to the HBV-DNA expression so as to reduce the risk of mother-to-infant transmission of HBV infection.
