日间手术模式下射频消融治疗下肢静脉曲张840例临床分析

摘　　要：目的:探讨日间手术模式下射频消融术(RFA)治疗下肢静脉曲张的短期临床效果。方法:收集2019年1月至2022年9月武汉大学中南医院心血管外科收治的840例C2~C6级下肢静脉曲张手术患者的临床资料,采用RFA联合泡沫硬化剂、点状剥脱治疗。于术后1月、术后3个月进行随访,主要终点为作用段静脉闭塞率,次要终点包括术后3个月的静脉临床严重程度评分(VCSS)、慢性静脉疾病生活质量问卷14(CIVIQ-14)评分。结果:所有患者术后1个月射频消融段的静脉闭塞率高达98.93%。随访期间,无患肢后遗症、肺栓塞等不良事件。术后3个月,患者的CIVIQ-14、AVVQ评分较术前明显改善,差异有统计学意义(P<0.01)。结论:日间手术模式下射频消融术(RFA)联合泡沫硬化剂、点状剥脱治疗下肢静脉曲张疗效满意,可行性强,具有临床推荐意义。Objective:To probe the short-term clinical effect of radiofrequency ablation(RFA)treatment for lower extremity varicose veins under day surgery mode.Methods:Clinical data of 840 patients with C2-C6 lower extremity varicose veins operated in Department of Cardiovascular Surgery of Zhongnan Hospital of Wuhan University from January 2019 to September 2022 were collected,and all the patients were treated with RFA combined with foam sclerosing agent and punctuate stripping.Follow-up was performed after 1 month and 3 months post-operation.The primary endpoint was the active vein occlusion rate,and the secondary endpoints included Venous Clinical Severity Score(VCSS)and Chronic Venous Insufficiency Questionnaire-14 Item(CIVIQ-14)at the end of 3 months after the operation.Results:The active vein occlusion rate was 98.93%counted from all patients with RFA treatment.During the follow-up,no serious adverse events such as limb sequela and pulmonary embolism were reported.At the end of 3 months after the operation,the VCSS and CIVIQ-14 scores decreased significantly(P<O.O1).Conclusion:RFA combined with foam sclerosing agent and punctuate stripping following day surgery model could achieve good effects on lower extremity varicose veins.The therapies are clinically recommended.
