早期血液灌流对急性有机磷农药中毒血清胆碱酯酶活力恢复效果及并发症分析

摘　　要：目的分析急性有机磷农药中毒病人应用早期血液灌流对血清胆碱酯酶活力恢复以及并发症的影响。方法以2020年6月-2022年6月为研究时间段,甄选急性有机磷农药中毒病人一共80例实施研究分析,分组方法选择随机数字表法,分为对照组和观察组,每组各40例,对照组采用非血液灌流治疗,观察组采用早期血液灌流治疗,比较两组患者昏迷时间、长托宁用量、氯解磷定剂量、机械通气时间、住院时间、血清胆碱酯酶活力治疗前后改善情况、并发症发生情况。结果①与对照组比较,观察组患者昏迷时间、长托宁用量、氯解磷定剂量、机械通气时间、住院时间显著较少,差异有统计学意义(P<0.05)。②治疗前,两组患者血清胆碱酯酶活力比较,无显著差异(P>0.05),治疗后,两组血清胆碱酯酶活力水平均升高,观察组高于对照组(P<0.05)。③消化道出血、呼吸衰竭、脑水肿、肺水肿等并发症发生率与对照组比较,观察组更低,差异显著(P<0.05)。结论应用早期血液灌流治疗急性有机磷农药中毒病人可获得显著效果,能够减少昏迷时间、长托宁用量、氯解磷定剂量、机械通气时间、住院时间,对病人血清胆碱酯酶活力具有改善作用,同时安全性较高,建议临床推广应用。Objective To study the effect of early hemoperfusion on the recovery of serum cholinesterase activity and complications in patients with acute organophosphorus pesticide poisoning.Methods From June 2020 to June 2022,a total of 80 patients with acute organophosphorus pesticide poisoning were selected for study and analysis.The analysis method was a random number table.They could be divided into control group and observation group,with 40 patients in each group.The control group was treated with non blood perfusion,while the observation group was treated with early blood perfusion.The comatose time,dosage of long tonin,fixed dosage of chlorophosphorus,duration of mechanical ventilation,length of hospitalization The improvement and complications of serum cholinesterase activity before and after treatment.Results©Compared with the control group,the comatose time,the dosage of Changtuoning,the dosage of chlorophosphorus,the duration of mechanical ventilation and the length of hospital stay in the observation group were significantly less than those in the control group(P<0.05).©Before treatment,there was no significant difference in the serum cholinesterase activity between the two groups(P>0.05).After treatment,the serum cholinesterase activity level was higher in the observation group than in the control group(P<0.05).©The incidence of gastrointestinal bleeding,respiratoryfailure,brain edema,pulmonary edema and other complications in the observation group was significantly lower than that in the control group(P<0.05).Conelusion The application of early hemoperfusion in the treatment of patients with acute organophosphorus pesticide poisoning can obtain outstanding and significant effects,reduce the time of coma,the dosage of long tolbutamide,the dosage of chlorophosphorus,the duration of mechanical ventilation,and the length of hospital stay.It can improve the activity of serum cholinesterase of patients,and has high safety.It is suggested that clinical application should be popularized.
