重复静脉注射钆类对比剂在大鼠脑内钆沉积的研究

摘  要:目的研究重复静脉注射不同分子类型含钆对比剂(gadolinium-based contrast agents,GBCAs)后大鼠脑内钆沉积情况。方法将24只雌性SD大鼠随机分为钆双胺组、钆特酸葡甲胺组、生理盐水组,每组各8只,分别静脉注射钆双胺(1.2 mmol/kg)、钆特酸葡甲胺(1.2 mmol/kg)、等量生理盐水,1次/d,连续注射10天。在最后1次注射结束后的1周和8周时对大鼠进行脑部磁共振成像(magnetic resonance imaging,MRI)扫描,并通过电感耦合等离子体质谱仪(inductively coupled plasma mass spectrometry,ICP-MS)测量脑组织内钆含量。结果MRI结果显示,钆双胺组和钆特酸葡甲胺组大鼠脑部T1加权象(T1 weighted image,T1WI)图像上小脑深部核团(deep cerebellar nuclei,DCN)信号强度(signal intensity,SI)/脑干SI和DCN SI/小脑实质SI在注射结束后的1周时均显著高于生理盐水组(P均<0.01);8周时,钆双胺组DCN SI/脑干SI仍高于生理盐水组(P均<0.05),而钆特酸葡甲胺组DCN SI/脑干SI与生理盐水组持平,组间比较差异无统计学意义(P>0.05)。1周、8周时,钆双胺组DCN SI/脑干SI、DCN SI/小脑实质SI均略高于钆特酸葡甲胺组,但组间比较差异无统计学意义(P>0.05)。ICP-MS结果显示,在两种GBCAs注射后1周和8周时大鼠脑内均发现有钆残留,且钆含量随着时间推移明显降低(P均<0.01),但两组间残留的钆含量差异无统计学意义(P>0.05)。结论给大鼠重复静脉注射GBCAs后,钆能够长期持续存在,MRI表现为T1WI图像上DCN信号明显增高,且随时间推移大鼠脑内钆含量显著降低。线型GBCAs钆双胺组钆含量残留略高于环型GBCAs钆特酸葡甲胺组,但无统计学差异。提示在决定使用GBCAs时,考虑残余钆的潜在未知风险,应选择合适的更加安全的对比剂。Objective To investigate the deposition of gadolinium in rat′s brain after repeated intravenous administration of gadolinium-based contrast agents(GBCAs).Methods A total of 24 female SD rats were randomly divided into gadodiamide group,gadoterate meglumine group and normal saline group,with 8 in each,which were injected intravenously with gadodiamide(1.2 mmol/kg),gadoterate meglumine(1.2 mmol/kg)or equivalent normal saline once a day for 10 days.Magnetic resonance imaging(MRI)brain scans were performed at 1 and 8 weeks after the final injection,gadolinium content in brain tissue was determined by inductively coupled plasma mass spectrometry(ICP-MS).Results MRI results showed that the deep cerebellar nucleus(DCN)signal intensity(SI)/brainstem SI and DCN SI/cerebellar parenchyma SI of the on T1 weighted image(T1WI)image of the brain of rats in the gadodiamide group and the gadoterate meglumine group were significantly higher than those in the normal saline group at 1 week after the end of injection(all P>0.01);at the 8th week,the DCN SI/brainstem SI ratio of gadodiamide group was still higher than that of normal saline group,and the difference between groups was statistically significant(all P<0.05),while the DCN SI/brainstem SI ratio of gadoterate meglumine group was the same as that of normal saline group,and there was no statistically significant difference between groups(P>0.05).At week 1 and week 8,DCN SI/brainstem SI and DCN SI/cerebellar parenchyma SI in gadodiamide group were slightly higher than those in gadoterate meglumine group,but there was no statistical significance between groups(P>0.05).The results of ICP-MS showed that gadolinium residue was found in the brain of rats at the 1st and 8th weeks after the injection of two GBCAs,and the content of gadolinium significantly decreased with the passage of time(all P<0.01),but there was no statistically significant difference between the two groups(P>0.05).Conclusion After repeated intravenous administration of GBCAs in rats,gadolinium persisted for a lo
