摘　　要：目的比较时差成像(time-lapse imaging,TLI)技术与体外受精-胚胎移植(in vitro fertilization and embryo transfer,IVF-ET)传统培养方法二者的培养效果,探讨TLI技术能否为胚胎发育及评估提供更有利的条件,从而改善妊娠结局。方法选取作者医院2022-01~03月行IVF-ET患者57例,为验证TLI系统的有效性及可行性,将每位患者的成熟卵子随机等分纳入TLI培养组(T1组)和传统体外培养组(C1组)。比较两组成熟卵子的2PN受精率、可移植胚胎率、优质胚胎率、囊胚形成率、优质囊胚率有无差别。随后收集2022-03~05月完全行TLI培养78个移植周期(T2组)及同时期传统体外培养146个移植周期(C2组)患者的资料,比较两组患者的胚胎指标及妊娠结局。结果T1组与C1组成熟卵子的2PN受精率、可移植胚胎率、优质胚胎率、囊胚形成率、优质囊胚率差异无统计学意义(P>0.05)。T2组与C2组患者各胚胎指标差异无统计学差异(P>0.05),但T2组患者的种植率、临床妊娠率显著高于C2组(P均<0.05),早期流产率显著低于C2组(P均<0.05)。结论TLI系统培养效果稳定,可将胚胎发育形态学与动力学相结合,选择发育潜能最优的胚胎进行移植,有效改善临床妊娠结局。Objective To compare the culture effects of time-lapse imaging(TLI)technology and in vitro fertilization and embryo transfer(IVF-ET)conventional culture methods,and to explore whether TLI technology can improve pregnancy outcomes by providing more advantageous conditions.Methods A total of 57 patients who underwent IVF-ET in author′s hospital from January to March 2022 were selected.In order to verify the effectiveness and feasibility of the TLI system,the mature ovum of each patient were randomly divided into two groups:TLI culture group(T1group)and conventional culture group(C1group).The 2PN fertilization rate,transferable embryo rate,high-quality embryo rate,blastocyst formation rate and high-quality blastocyst rate of mature ovum were compared between the two groups.After that,patients′information,concluding 78 transplantation cycles with completely TLI culture(T2group)from March to May 2022 and 146 transplantation cycles with conventional culture(C2group)were collected during the same time,and the embryo parameters and pregnancy outcomes of the two groups were compared.Results There were no significant differences of 2PN fertilization rate,transferable embryo rate,high-quality embryo rate,blastocyst formation rate and high-quality blastocyst rate of mature ovum between T1and C1groups(P>0.05).There were no significant difference in the embryo parameters between the T2and C2groups(all P>0.05),but the implantation rate and clinical pregnancy rate in the T2group were significantly higher than those in the C2group(all P<0.05),and the early miscarriage rate was significantly lower than that in the C2group(all P<0.05).Conclusion The TLI system has a stable culturing effects.It can combine embryonic morphology and dynamics to select embryos with the best developmental potential for transplantation,which can effectively improve clinical pregnancy outcomes.
