摘 要:目的探讨两种鼻腔冲洗方式在鼻咽癌放射治疗患者中的应用效果。方法选取2020年4月至2021年5月于浙江省肿瘤医院行鼻咽癌根治性放疗的患者60例,依据随机数字表法将其分为两组,每组各30例。A组患者采用雾化式鼻腔冲洗器,B组患者采用气水式鼻腔冲洗器,分别于放疗前、中、后期比较两组患者的鼻腔黏膜反应、鼻腔鼻窦结局测试20(sino-nasal outcome test-20,SNOT-20)量表评分及冲洗舒适度。结果放疗前期、中期两组患者的鼻腔黏膜反应程度比较差异无统计学意义(P>0.05),放疗后期,A组患者的鼻腔黏膜反应程度显著高于B组(P<0.05);放疗期间,A组患者的SNOT-20量表总分和鼻部症状、相关症状评分显著高于B组(P<0.05),两组患者的睡眠障碍、情感结局评分比较差异无统计学意义(P>0.05);A组患者的洗鼻压力、呛水、鼻黏膜刺激评分均显著低于B组(P<0.05),清洁度评分显著高于B组(P<0.05)。结论使用气水式鼻腔冲洗器进行鼻腔冲洗,患者鼻腔黏膜反应程度、生存质量及清洁度均优于雾化式鼻腔冲洗器,但使用雾化式鼻腔冲洗器更为方便,患者舒适度更高。Objective To investigate the effect of two nasal irrigation methods in radiotherapy of nasopharyngeal carcinoma.Methods A total of 60 patients with nasopharyngeal carcinoma who underwent radical radiotherapy in Zhejiang Cancer Hospital from April 2020 to May 2021 were selected and divided into two groups according to the random number table method,with 30 cases in each group.The aerosol nasal irrigator was used in group A,and the air-water nasal irrigator was used in group B.The nasal mucosa reaction,the sino-nasal outcome test-20(SNOT-20)score and flushing comfort level were compared between the two groups at the early stage,middle stage and late stage of radiotherapy.Results There was no significant difference in the degree of nasal mucosal reaction in early and middle stage of radiotherapy between the two groups(P>0.05).The degree of nasal mucosal reaction in late stage of radiotherapy in group A was higher than that in group B(P<0.05).During radiotherapy,the total score of SNOT-20 scale and scores of nasal symptoms and related symptoms in group A were significantly higher than those in group B(P<0.05).There was no significant difference in sleep disorders and emotional outcome scores between the two groups(P>0.05).The scores of nasal washing pressure,water choking and nasal mucosal irritation in group A were significantly lower than those in group B(P<0.05),and the score of cleanliness was significantly higher than that in group B(P<0.05).Conclusion Using air-water nasal irrigator for nasal irrigation,the degree of nasal mucosal reaction,quality of life and cleanliness of patients are better than aerosol nasal irrigator,but use of aerosol nasal irrigator is more convenient and patients’comfort is higher.
