摘  要:目的探究血清和肽素(Copeptin)、B型尿钠肽(BNP)水平变化与慢性心力衰竭(CHF)患者短期不良终点事件的相关性,并分析二者联合检测预测短期不良终点事件的效能。方法选取2019年2月至2021年9月许昌龙耀医院收治的238例CHF患者为研究对象,出院后随访3个月,根据是否发生短期不良终点事件分为事件组、无事件组,均于入院后第2天检测血清Copeptin、BNP水平,比较两组基线资料,分析CHF患者短期不良终点事件发生的影响因素,以受试者工作特征(ROC)曲线分析血清Copeptin、BNP水平预测CHF患者短期不良终点事件发生的效能。结果事件组血清Copeptin、BNP水平均较无事件组高,差异有统计学意义(P<0.05);多因素Logistic回归分析结果显示,Copeptin、BNP水平升高是CHF患者短期不良终点事件发生的危险因素(P<0.05);血清Copeptin、BNP水平与CHF患者短期不良终点事件发生呈正相关(r=0.612、0.704,P<0.05);ROC曲线分析结果显示,血清Copeptin、BNP联合检测预测CHF患者短期不良终点事件发生的曲线下面积为0.903,灵敏度为90.12%,特异度为87.26%,均高于二者单独检测(P<0.05)。结论血清Copeptin、BNP水平与CHF患者短期不良终点事件发生风险密切相关,血清Copeptin、BNP联合检测可为CHF患者预后评估提供依据。Objective To explore the correlations between serum levels of Copeptin,B-type natriuretic peptide(BNP)and short-term adverse endpoint events in patients with chronic heart failure(CHF),and to analyze the efficacy of both of them in predicting short-term adverse endpoint events.Methods A total of 238 CHF patients admitted to Xuchang Longyao Hospital from February 2019 to September 2021 were selected and followed up for 3 months after discharge.According to the occurrence of short-term adverse endpoint events they were divided into event group and no event group.Serum Copeptin and BNP levels were detected and compared on the 2nd day after admission,and clinical data of the two groups were compared.The factors influencing the occurrence of short-term adverse endpoint events in CHF patients were analyzed,and the efficacy of serum Copeptin and BNP levels in predicting the occurrence of short-term adverse endpoint events in patients with CHF was analyzed by receiver operating characteristic(ROC)curve.Results The levels of serum Copeptin and BNP in event group were higher than those in no event group,and the differences were statistically significant(P<0.05).Multivariate Logistic regression analysis showed that increased Copeptin,BNP were the risk factors for short-term adverse endpoint events in patients with CHF(P<0.05).Serum Copeptin and BNP levels correlated positively with short-term adverse endpoint events in CHF patients(r=0.612,0.704,P<0.05).ROC curve analysis results showed that the area under the curve of the combined detection of serum Copeptin and BNP to predict the occurrence of short-term adverse endpoint events in CHF patients was 0.903,the sensitivity was 90.12%,and the specificity was 87.26%,which were all higher than those of single detection(P<0.05).Conclusion Serum Copeptin and BNP levels relate closely to the risk of short-term adverse endpoint events in patients with CHF.The combined detection of serum Copeptin and BNP could provide a reasonable basis for the prognosis evaluation and improvement of
