摘　　要：目的探讨子宫肌瘤患者行经阴道子宫肌瘤剔除术手术失败的影响因素。方法选取2018年1月—2022年12月于三明市第二医院诊断为子宫肌瘤并应用经阴道子宫肌瘤剔除术治疗的96例患者作为研究对象。收集患者资料,记录手术结果,术中经阴道无法进入腹腔、经阴道无法将子宫肌瘤切除、术中出血量多、损伤周围脏器等因素造成无法经阴道完成手术,需更改其他手术方式完成者纳入失败组;可经阴道完成手术,手术经阴道进入腹腔顺利,无周围脏器损伤,术中将子宫肌瘤剔除,出血量不多,无需更改其他途径手术者纳入成功组。分析手术失败的影响因素。结果成功组与失败组比较,年龄差异无统计学意义(P>0.05),而是否生育、子宫肌瘤大小、子宫肌瘤数量、是否剖宫产史差异有统计学意义(P<0.05)。子宫肌瘤数量、子宫肌瘤大小、是否剖宫产会对失败产生显著的正向影响关系。是否生育会对成败产生显著的负向影响关系。结论手术失败与是否生育、子宫肌瘤大小、子宫肌瘤数量,既往是否剖宫产有关。Objective To explore the influencing factors of failure of transvaginal myomectomy in patients with uterine fibroids.Methods From January 2018 to December 2022,a total of 96 patients diagnosed with uterine fibroids in the Sanming Second Hospital were treated by transvaginal myomectomy and selected as research subjects,patient data was collected,surgical results were recorded,and menstruation those who could not enter the abdominal cavity through the vagina during surgery,could not remove uterine fibroids transvaginally,had a large amount of intraoperative bleeding,and damaged surrounding organs,and needed to change other surgical methods to complete the operation were included in the failure group.The operation can be completed vaginally,the operation enters the abdominal cavity smoothly through the vagina,there was no damage to the surrounding organs,the uterine fibroids were removed during the operation,the amount of bleeding was not much,and there was no need to change other routes,and the surgeons were included in the successful group.The factors influencing surgical failure were analyzed.Results Compared with the successful group,there was no significant significance in age between the successful group and the failed group(P>0.05),but the size of uterine fibroids,the number of uterine fibroids,and the history of cesarean section were statistically significant(P<0.05).Number of fibroids,fibroid size,cesarean section can have a significant positive impact on failure.In addition,whether giving birth will have a significant negative impact on failure.Conclusion Surgical failure is related to fertility,size and number of uterine fibroids,and previous cesarean section.
