摘 要:目的观察肋间肌增厚分数联合膈肌浅快呼吸指数对撤机的预测价值。方法纳入2020年7月—2021年12月入住福建医科大学附属泉州第一医院重症医学科60例机械通气患者。按照撤离机械通气标准将纳入研究的患者分为成功组与失败组。分析两组患者基本情况指标、呼吸相关指标、超声相关数据指标的统计学差异;分析肋间肌增厚分数联合膈肌浅快呼吸指数、浅快呼吸指数两种方法的曲线下面积及准确率;分析曲线下面积及准确率的统计学差异。结果60例患者中,共有18例患者撤机失败。浅快呼吸指数预测撤机效能的曲线下面积为0.664,95%CI(0.476~0.852),准确率为63.56%。肋间肌增厚分数联合膈肌浅快呼吸指数预测撤机的曲线下面积为0.862,95%CI(0.764~0.961),准确率为82.62%。两组曲线下面积、准确率差异有统计学意义(P<0.05)。两组曲线下面积、准确率差异有统计学意义(P<0.05)。结论肋间肌增厚分数联合膈肌浅快呼吸指数可有效预测撤机结果。Objective To observe the predictive value of intercostal muscle thickening fraction combined with diaphragm shallow fast respiratory index.Methods A total of 60 mechanical ventilation patients were admitted to department of critical care medicine of Quanzhou First Hospital Affiliated to Fujian Medical University from July 2020 to December 2021.Patients enrolled in the study were divided into successful and failed groups according to evacuation mechanical ventilation criteria.The statistical differences of basic information,respiration and ultrasound data between the two groups of the intercostal muscle thickening score and the area and accuracy of the area and accuracy were analyzed.Results A total of 18 patients out of 60 patients failed in machine withdrawal.The area under curve of the rapid shallow breath index was 0.664,95%CI(0.476-0.852)with an accuracy of 63.56%.The area under curve of intercostal muscles thickening fraction combined with diaphragm rapid shallow breathing index was 0.862,95%CI(0.764-0.961)with an accuracy of 82.62%.The area and accuracy of the two groups were statistically significant(P<0.05).Conclusion The intercostal muscle thickening fraction combined with the diaphragm shallow fast breathing index can predict the withdrawal results effectively.
