摘  要:目的探讨小剂量多层螺旋CT重组技术结合电子纤维支气管镜对小儿气管支气管异物的诊断价值。方法选取2019年1月至2022年1月金华市人民医院收治的气管支气管异物患儿188例作为研究对象,另选择金华市人民医院接受健康体检患儿188例作为对照组,入选患者均行小剂量(multislice spiral CT,MSCT)重组技术、电子纤维支气管镜检测,比较异物种类的分布特征,评估小剂量MSCT重组技术、电子纤维支气管镜对于小儿气管支气管异物的诊断价值。结果各年龄患儿植物性气管支气管异物118例(62.76%),动物性气管支气管异物53例(28.19%),化学合成品气管支气管异物17例(9.04%)。小剂量MSCT重组技术检测阳性预测值和阴性预测值分别为95.26%、91.37%,电子纤维支气管镜检测阳性预测值和阴性预测值分别为97.08%、94.87%,低于两项联合检测的99.45%、96.89%(P<0.05);小剂量MSCT重组技术检测方法的敏感度、特异性、准确度分别为85.63%、95.74%、90.69%,电子纤维支气管镜检测方法的敏感度、特异性、准确度分别为88.30%、97.34%、91.76%,显著低于两项联合检测的96.80%、99.46%、98.13%(P<0.05)。结论使用小剂量MSCT重组技术联合电子纤维支气管镜对小儿气管支气管异物具有较高的诊断价值。Objective To explore the diagnostic value of low-dose multilayer spiral CT(MSCT)recombinant technology combined with electronic fiber bronchoscopy in pediatric tracheobronchial foreign body.Methods A total of 188 children with tracheobronchial foreign bodies treated in Jinhua People’s Hospital from January 2019 to January 2022 were selected as the research subjects,188 children received physical examination in Jinhua People’s Hospital were selected for the control group,low-dose MSCT recombinant technique and electronic fiber bronchoscopy were performed in all subjects.The distribution characteristics of foreign bodies in all children were compared.The distribution characteristics of foreign body types,and evaluated the diagnostic value of low-dose MSCT reconstitution technology and electronic fiberoptic bronchoscopy for tracheobronchial foreign bodies in children were compared.Results The largest number of plant tracheobronchial foreign bodies in children of all ages,118(62.76%)cases,animal tracheobronchial foreign bodies were followed 53(28.19%)cases,tracheobronchial foreign body was 17(9.04%).The positive and negative predictive values of the low-dose MSCT recombinant technology test were 95.26% and 91.37%,respectively.The positive and negative predictive values of electron fiber bronchooptic bronchoscopy were 97.08% and 94.87%,respectively,less than the 99.45% and 96.89% of the two combined tests(P<0.05);The sensitivity,specificity,and accuracy of the low-dose MSCT recombinant technology detection technique were 85.63%,95.74%,and 90.69%,the sensitivity,specificity,and accuracy of the electronic fiber bronchooptic bronchoscopy detection method were 88.30%,97.34%,91.76%,it was significantly lower than the 96.80%,99.46%,and 98.13% of the two combined tests(P<0.05).Conclusion The use of a small-dose MSCT recombination technology combined with electronic fiber optic bronchoscopy has a high diagnostic value for pediatric tracheobronchial foreign bodies.
