摘 要:目的估算北京市2018—2019年度流感流行季,流感疫苗接种对预防住院严重急性呼吸道感染(severe acute respiratory infection,SARI)的效果。方法以北京市11家哨点医院的SARI病例为研究对象,通过疫苗接种登记系统获得流感疫苗接种史,通过RT-PCR检测判定流感感染情况。利用非条件logistic回归调整年龄、发病时间和基础病的影响,计算流感核酸阳性组与阴性组SARI患者流感疫苗接种的比值比(odds ratio,OR),用1-OR计算疫苗保护效果(vaccine effectiveness,VE)。结果疫苗总体VE估计值为-42.6%(95%CI:-152.7%~19.5%),5~59岁组VE估计值为58.5%(-119.4%~92.2%),接种60 d以内VE估计值为15.8%(-142.8%~70.8%)。结论2018—2019流感流行季北京市流感疫苗接种对于预防流感引起的SARI保护效果有限,应提高重点人群接种率。Objective To estimate the vaccine effectiveness(VE)of the seasonal influenza vaccination in prevention of hospitalized severe acute respiratory infection(SARI)in Beijing in the 2018/2019 epidemic season of influenza.Methods Hospitalized SARI cases in 11 Beijing hospitals during 2018/2019 epidemic season of influenza were selected as study subjects.Vaccination status was determined from registry record system.Influenza virus infection was detected by RT-PCR.A binary logistic regression model was used to adjust influence from age,time of onset and underlying diseases to calculate the odds ratio(OR)between influenza virus nucleic acid positive and negative groups.VE was then calculated by 1-OR.Results The overall estimated VE was-42.6%(95%CI:-152.7%to 19.5%)while VE of 5-59 group was 58.5%(-119.4%to 92.2%).VE of vaccination within 60 days was 15.8%(-142.8%to 70.8%).Conclusions The protection effectiveness of influenza vaccine for hospitalized SARI patients in Beijing during 2018/2019 epidemic season of influenza were limited.Vaccination coverage in target groups should be enhanced.
