摘　　要：目的:分析天津市二、三级医院老年护理服务情况,提出改进策略。方法:基于天津市88家二、三级医院的问卷调查数据,构建老年护理服务评价指标,并使用熵值法确定指标权重,进而对天津市二、三级医院老年护理服务能力、老年护理服务资源、老年护理服务规范水平及三者间的耦合协调度进行分析。结果:天津市二、三级医院老年护理服务能力平均分为0.26,老年护理服务资源平均分为0.09,老年护理服务规范平均分为0.35,老年护理服务综合得分平均分为0.23。天津市三级医院老年护理服务能力水平、老年护理服务资源水平、老年护理服务规范水平和老年护理服务综合水平均显著高于二级医院。88家医院中,协调类型为良好协调的医院有1家,协调类型为中级协调的医院有15家,协调类型为勉强协调的医院有20家,协调类型为中度失调的医院有32家,协调类型为严重失调的医院有20家。结论:建议进一步加强二、三级医院老年护理服务工作,推广老年护理服务典型经验,化解阻碍老年护理服务发展的瓶颈问题,强化老年护理服务质量控制和行为监管。OBJECTIVE To analyze the situation of elderly nursing service in secondary and tertiary hospitals in Tianjin,and to propose improvement strategies.METHODS Based on the questionnaire survey data of 88 secondary and tertiary in Tianjin,the evaluation indicators of elderly nursing services were extracted,and the entropy method was used to determine the weight of the indicators.The level and the coupling coordination degree between the three are analyzed.RESULTS The average score of geriatric nursing service ability was 0.26,the average score of geriatric nursing service resources was 0.09,the average score of geriatric nursing service specification was 0.35,and the average score of geriatric nursing service comprehensive score was 0.23.The level of geriatric nursing service capability,senior nursing service resources,geriatric nursing service standardization,and geriatric nursing service comprehensive level of Tianjin tertiary medical institutions were significantly higher than those of secondary medical institutions.Among the 88 medical institutions,there is 1 medical institution with good coordination type,15 medical institutions with medium coordination type,20 medical institutions with barely coordination type,and moderately disordered medical institution.There are 32institutions,and 20 medical institutions with severe disorders of coordination type.CONCLUSION It is recommended to further strengthen the work of geriatric nursing service,popularize the typical experience of geriatric nursing service,resolve the bottleneck problem that hinders the development of geriatric nursing service,and strengthen the quality control and behavior supervision of geriatric nursing service.
