坤宁安加减治疗围绝经期综合征情绪障碍的临床研究

摘　　要：目的:观察坤宁安加减治疗围绝经期综合征情绪障碍的临床疗效及安全性。方法:将符合纳入标准的60例围绝经期综合征情绪障碍患者依据随机数字表随机分为坤宁安组和对照组,每组30例。对照组给予舒肝解郁胶囊,每次2粒,每日2次;坤宁安组给予坤宁安随症加减,每日1剂,早晚饭后口服;两组均连续治疗1个月。记录两组患者治疗前后汉密尔顿抑郁量表(HAMD)、汉密尔顿焦虑量表(HAMA)、Kupperman绝经指数(KMI)、绝经期生存质量量表(MENQOL)评分的变化情况,并检测血清E_(2)和FSH水平。结果:治疗后,两组患者HAMD-17、HAMA、KMI、MENQOL评分均较治疗前显著降低(P<0.05),且与对照组比较,坤宁安组HAMD-17、HAMA、MI、MENQOL评分更低(P<0.05);两组患者治疗后血清E_(2)水平均升高(P<0.05),FSH水平降低(P<0.05),且坤宁安组血清E_(2)水平高于对照组(P<0.05),FSH水平低于对照组(P<0.05);两组患者在治疗过程中均未有不良反应发生情况。结论:坤宁安可有效改善围绝经期女性激素水平,减轻围绝经期情绪障碍状况,提高生活质量。Objective:To observe the clinical efficacy and safety of modified Kunning An in treatment of emotional disorders of perimenopausal syndrome.Methods:60 patients with mood disorders of perimenopausal syndrome,who met the inclusion criteria,were randomly divided into the Kunning An group and control group based on a random number table,with 30 patients in each group.Shugan Jieyu Capsules were given to the control group,2 capsules a time,twice a day;Kunning An group was given orally the decoction of Kunning An which is modified according to patients’symptoms,1 dose per day,after breakfast and supper.Both groups were treated continuously for one month.We recorded the score changes in Hamilton Rating Scale for Depression(HAMD),Hamilton Anxiety Scale(HAMA),Kupperman menopausal index(KMI),and menopause-specific quality of life questionnaire(MENQOL)of the two groups before and after the treatment,and detected E_(2)and FSH serum levels.Results:After the treatment,the HAMD-17,HAMA,KMI,and MENQOL scores of both groups were significantly lower than those before the treatment(P<0.05);and compared with the control group,the HAMD-17,HAMA,KMI,and MENQOL scores of the Kunning An group were lower(P<0.05).After the treatment,the E_(2)serum levels of both groups increased(P<0.05),while FSH levels decreased(P<0.05).Moreover,the E_(2)serum levels in Kunning An group were higher than those in the control group(P<0.05),but FSH levels were lower than those in the control group(P<0.05).Patients in both groups did not experience any adverse events during the treatment process.Conclusion:Kunning An can effectively improve hormone levels,alleviate emotional disorders,and improve quality of life for perimenopausal women.
