射频消融联合泡沫硬化剂闭合及点状剥脱术治疗大隐静脉曲张的效果观察

摘　　要：目的分析射频消融联合泡沫硬化剂闭合及点状剥脱术治疗大隐静脉曲张的效果。方法抽取2019年5月至2022年5月新乡市第二人民医院收治的104例大隐静脉曲张患者作为研究对象,采用随机抽样法将其分为对照组及研究组,每组52例。对照组行大隐静脉高位结扎+曲张静脉剥脱术治疗,研究组行射频消融联合泡沫硬化剂闭合、点状剥脱术治疗。比较两组治疗前后静脉临床严重程度评分(VCSS)及慢性静脉功能不全问卷(CIVIQ)评分,比较两组围术期相关指标、术后疼痛评分及术后并发症发生情况。结果治疗前,两组VCSS、CIVIQ评分比较差异未见统计学意义(P>0.05);治疗后,研究组VCSS低于对照组,CIVIQ评分高于对照组(P<0.05)。研究组切口数量少于对照组、住院时间短于对照组,术中出血量少于对照组(P<0.05)。术后2、6、12、24 h,研究组疼痛程度评分均低于对照组(P均<0.05)。研究组术后并发症发生率(1.92%,1/52)低于对照组(13.46%,7/52),P<0.05。结论射频消融联合泡沫硬化剂闭合及点状剥脱术治疗大隐静脉曲张效果确切且安全性好。Objective To analyze the effect of radiofrequency ablation combined with foam sclerosing agent closure and punctate stripping in the treatment of great saphenous varicose veins.Methods A total of 104 patients with great saphenous varicose veins admitted to Xinxiang Second People’s Hospital from May 2019 to May 2022 were selected as the study subjects,and they were divided into control group and study group according to random sampling method,with 52 cases in each group.The control group was treated by high ligation of great saphenous vein and varicose vein stripping,and the study group was treated by radiofrequency ablation combined with foam sclerosing agent closure and punctate stripping.The venous clinical severity score(VCSS)and chronic venous insuff iciency questionnaire(CIVIQ)score before and after treatment were compared between the two group.And the perioperative related indexes,postoperative pain score and postoperative complications were compared between the two groups.Results There was no significant difference in VCSS and CIVIQ score between the two groups before treatment(P>0.05);after treatment,the VCSS of the study group was lower than that of the control group,and the CIVIQ score was higher than that of the control group(P<0.05).The number of incisions in the study group was less than that in the control group,the hospital stay was shorter than that in the control group,and the intraoperative volume was less than that in the control group(P<0.05).The pain degree score of the study group was lower than that of the control group at 2,6,12 and 24 hours after surgery(all P<0.05).The incidence of postoperative complications in the study group(1.92%,1/52)was lower than that in the control group(13.46%,7/52),P<0.05.Conclusions Radiofrequency ablation combined with foam sclerosing agent closure and punctate stripping is effective and safe in the treatment of great saphenous varicose veins.
