摘 要:目的分析多层螺旋CT和X线诊断脊柱侧弯的价值。方法抽取2021年1月至2021年12月南阳市中心医院收治的脊柱侧弯患者72例,均行多层螺旋CT和X线检查,比较侧弯Cobb角检查结果、病变检出情况及影像满意度。结果多层螺旋CT诊断单一畸形、半椎体畸形、裂椎、脊柱开裂侧弯Cobb角小于X线诊断(P<0.05)。多层螺旋CT共检出429处病变,其中半椎体38处,蝴蝶椎56处,分节不良132处,脊柱裂161处,脊髓纵裂4处,肋骨38处;X线共检出81处病变,其中半椎体13处,蝴蝶椎2处,分节不良24处,脊柱裂14处,脊髓纵裂1处,肋骨27处。多层螺旋CT显示侧弯全貌、顶椎、脊髓畸形影像满意度(98.61%,71/72;100.00%,72/72;73.61%,53/72)高于X线(58.33%,42/72;84.72%,61/72;1.39%,1/72),P<0.05。结论与多层螺旋CT相比,X线诊断脊柱侧弯Cobb角的效果较好,但多层螺旋CT能清晰显示脊柱侧弯整体结构和病变情况,且影像学满意度高,临床可结合患者实际情况将多层螺旋CT和X线联合应用于脊柱侧弯诊断中。Objective To analyze the application value of multi-slice spiral CT and X-ray in the diagnosis of scoliosis.Methods A total of 72 patients with scoliosis treated in Nanyang Central Hospital from January 2021 to December 2021 were selected.And all of them underwent multi-slice spiral CT and X-ray examinations.The results of Cobb angle examination,lesion detection and imaging satisfaction were compared.Results The Cobb angle of single vertebra deformity,hemivertebra deformity,vertebra dehiscence,and spinal bifida diagnosed by multi-slice spiral CT were smaller than those diagnosed by X-ray(P<0.05).Multi-slice spiral CT detected out a total of 429 lesions,including 38 cases hemivertebrae,56 cases butterfly vertebrae,132 cases poor segmentation,161 cases spinal bifida,4 cases longitudinal fissures,and 38 cases lesion in ribs.X-rays detected out a total of 81 lesions,including 13 cases hemivertebrae,2 cases butterfly vertebrae,24 cases poor segmentation,14 cases spinal bifida,1 case longitudinal fissure,and 27 cases lesion in ribs.The satisfaction of image of scoliosis,apical vertebra and spinal cord deformity detected by multi-slice spiral CT(98.61%,71/72;100.00%,72/72;73.61%,53/72)were higher than those detected by X-ray(58.33%,42/72;84.72%,61/72;1.39%,1/72),P<0.05.Conclusions Compared with multi-slice spiral CT,X-ray has a better effect in diagnosing Cobb angle of scoliosis,but multi-slice spiral CT can clearly display the overall structure and lesions of scoliosis,and the imaging satisfaction is high.In this case,the combination of multi-slice spiral CT and X-ray can be used in the diagnosis of scoliosis according to with the actual situation of patients.
