摘　　要：目的 探讨趋化因子c-c配体2 (CCL2)、细胞间黏附分子-1 (ICAM-1)在2型糖尿病(T2DM)伴慢性牙周炎(CP)患者血清中的表达及其与牙周病变程度的相关性。方法 选取2021年1月至2022年6月铜川市妇幼保健院和榆林市星元医院收治的80例T2DM伴CP患者为研究对象(T2DM+CP组),选取上述医院收治的80例单纯T2DM患者为单纯T2DM组,80例单纯CP患者为单纯CP组。收集三组患者的一般资料,采用酶联免疫吸附法检测并比较三组患者的血清CCL2、ICAM-1表达水平,采用Pearson法分析CCL2、ICAM-1与临床指标的相关性,Spearman法分析CCL2、ICAM-1与牙周病变程度的相关性。结果 T2DM+CP组患者的血清CCL2、ICAM-1水平分别为(57.30±10.48) pg/mL、(649.83±65.51) ng/mL,明显高于单纯CP组[(20.95±6.87) pg/mL、(462.23±50.80) ng/mL]和单纯T2DM组[(42.89±8.91) pg/mL、(519.92±56.22) ng/mL],差异均有统计学意义(P<0.05);CCL2、ICAM-1表达水平随着牙周病变程度的加重逐渐升高,差异具有统计学意义(P<0.05);经Spearman相关性分析结果显示,T2DM伴CP患者血清中CCL2、ICAM-1表达与牙周病变程度呈显著正相关(r=0.717,r=0.853,P<0.05);经Pearson相关性分析结果显示,T2DM+CP组患者血清中CCL2、ICAM-1表达与附着丧失(AL)、牙周袋探针深度(PD)、牙龈沟出血指数(SBI)、空腹血糖(FPG)、肿瘤坏死因子-α (TNF-α)、白介素-6 (IL-6)呈显著正相关(P<0.05)。结论 CCL2、ICAM-1在T2DM伴CP患者血清中高表达,两者与患者牙周病变程度密切相关。Objective To investigate the expression of chemokine c-c ligand 2(CCL2)and intercellular adhe-sion molecule-1(ICAM-1)in serum of patients with type 2 diabetes mellitus(T2DM)and chronic periodontitis(CP),and to analyze their correlation with the degree of periodontal disease.Methods From January 2021 to June 2022,80 patients with T2DM and CP who were admitted to Shaanxi Tongchuan Maternal and Child Health Care Hospital and Yulin Xingyuan Hospital were regarded as the research objects(T2DM+CP group).In addition,80 patients with simple T2DM in the two hospitals were regarded as the simple T2DM group,and 80 patients with simple CP were regarded as the simple CP group.General information was collected.The expression levels of CCL2 and ICAM-1 in serum were de-tected by enzyme-linked immunosorbent assay.Pearson correlation analysis was performed to analyze the correlation be-tween CCL2,ICAM-1 and clinical indicators,and Spearman correlation analysis was used to analyze the correlation be-tween CCL2 and ICAM-1 and the degree of periodontal lesions.Results The serum levels of CCL2 and ICAM-1 in the T2DM+CP group were(57.30±10.48)pg/mL,(649.83±65.51)ng/mL,significantly higher than(20.95±6.87)pg/mL,(462.23±50.80)ng/mL in the simple CP group and(42.89±8.91)pg/mL,(519.92±56.22)ng/mL in the simple T2DM group(P<0.05);the expression levels of CCL2 and ICAM-1 gradually increased with the aggravation of periodontal dis-ease(P<0.05).Spearman correlation analysis showed that the expression of CCL2 and ICAM-1 in serum of T2DM pa-tients with CP were positively correlated with the degree of periodontal disease(r=0.717,r=0.853,P<0.05).Pearson cor-relation analysis showed that serum levels of CCL2 and ICAM-1 in T2DM+CP group were significantly positively corre-lated with attachment loss(AL),periodontal pocket probe depth(PD),gingival bleeding index(SBI),fasting blood glu-cose(FPG),tumor necrosis factor-α(TNF-α)and interleukin-6(IL-6),P<0.05.Conclusion CCL2 and ICAM-1 are highly expressed in the serum of T2DM patients with CP,whic
