探讨外固定支架联合股前外侧肌皮瓣治疗GustiloⅢB/C型开放性胫腓骨折的疗效

摘　　要：目的研究外固定支架联合股前外侧肌皮瓣治疗GustiloⅢB/C型开放性胫腓骨折的疗效。方法方便选取2018年1月—2021年6月广州市番禺区第二人民医院收治的94例开放性胫腓骨折患者为研究对象,采用随机数表法分为两组。对照组47例接受钢板螺钉内固定联合股前外侧肌皮瓣治疗,观察组47例接受外固定支架联合股前外侧肌皮瓣治疗,比较两组患者治疗效果。结果观察组治疗优良率(70.21%),明显高于对照组(48.94%),差异有统计学意义(χ^(2)=4.417,P<0.05)。两组术后当天CRP、ESR、WBC水平均低于术前1 d,且观察组术后当天各指标水平均低于对照组,差异有统计学意义(P<0.05)。观察组术后6个月内并发症发生率为10.64%,明显低于对照组的27.66%,差异有统计学意义(χ^(2)=4.398,P<0.05)。结论外固定支架联合股前外侧肌皮瓣治疗GustiloⅢB/C型开放性胫腓骨折可获得更为满意的效果,治疗优良率更高,术后并发症更少。Objective To investigate the effect of external fixation stent combined with anterolateral femoral musculo⁃cutaneous flap on GustiloⅢB/C open tibiofibular fracture.Methods 94 patients with open tibiofibular fractures admit⁃ted to Panyu District Second People's Hospital from January 2018 to June 2021 were conveniently selected as the re⁃search objects,and randomly divided into two groups by number table method.47 cases in the control group received plate screw internal fixation combined with anterolateral femoral musculocutaneous flap,and 47 cases in the observa⁃tion group received external fixation stent combined with anterolateral femoral musculocutaneous flap.The therapeutic effect of the two groups was compared.Results The rate of excellent and good treatment effect in the observation group was 70.21%,which was significantly higher than 48.94%in the control group,and the difference was statistically sig⁃nificant(χ^(2)=4.417,P<0.05).The levels of CRP,ESR and WBC in the two groups on the day after surgery were lower than those on the day before surgery,and the levels of all indexes in the observation group were lower than those in the control group on the day after surgery,the difference was statistically significant(P<0.05).The complication rate in the observation group was 10.64%,significantly lower than 27.66%in the control group,and the difference was sta⁃tistically significant(χ^(2)=4.398,P<0.05).Conclusion The treatment of GustiloⅢB/C open tibiofibular fracture with ex⁃ternal fixation stent combined with anterolateral femoral musculocutaneous flap can achieve more satisfactory results,with higher rate of good treatment and less postoperative complications.
