摘 要:目的 探讨老年2型糖尿病患者认知功能与糖化白蛋白(GA)及糖化血红蛋白(HbA1c)比值(GA/HbA1c)的相关性。方法 选择2019年1~12月于首都医科大学宣武医院老年综合科就诊的161例老年2型糖尿病患者作为研究对象,运用简易精神状态评价量表(MMSE)评估认知功能情况,根据MMSE得分将患者分为认知功能障碍组(41例)及认知功能正常组(120例)。比较两组一般资料及实验室检查结果的差异,计算年龄、HbA1c、GA、GA/HbA1c与MMSE的相关性,logistic回归分析认知功能的独立影响因素,ROC曲线对GA/HbA1c在老年2型糖尿病认知功能障碍中的诊断价值进行评估。结果 认知功能障碍组的年龄与GA/HbA1c均高于认知功能正常组(P<0.05)。MMSE得分与年龄及GA/HbA1c均呈负相关(P<0.05)。logistic回归分析显示,年龄与GA/HbA1c均为影响老年2型糖尿病患者认知功能障碍的独立危险因素(P<0.05)。GA/HbA1c诊断老年2型糖尿病患者认知功能障碍的ROC曲线下面积为0.669(95%CI:0.575~0.763,P<0.05)。结论 GA/HbA1c与老年2型糖尿病患者认知功能障碍相关且易于获得,对认知功能障碍具有一定的预测能力。Objective To investigate the correlation between cognitive function and the ratio of glycated albumin to glycated hemoglobin(GA/HbA1c)in older patients with type 2 diabetes mellitus(T2DM).Methods A total of 161 older T2DM patients visited to Xuanwu Hospital of Capital Medical University from January to December 2019 were selected.The cognitive function was evaluated using the Mini-mental State Examination(MMSE).The patients were divided into a cognitive dysfunction group(41 cases)and a normal cognitive function group(120 cases)based on MMSE score.The general data and laboratory data were compared between the two groups.The correlations of MMSE score with age,HbA1c,GA and GA/HbA1c were analyzed.Logistic regression was used to analyze independent factors for cognitive function.Receiver operating characteristic(ROC)curve was used to evaluate the diagnostic value of GA/HbA1c for cognitive dysfunction in older T2DM patients.Results The cognitive dysfunction group had significantly higher age and GA/HbA1c than the normal cognitive function group(P<0.05).Age and GA/HbA1c were both negatively correlated with MMSE score(P<0.05).Logistic regression analysis showed that the age and GA/HbA1c were independent risk factors for cognitive dysfunction in older T2DM patients.ROC curve analysis found that the area under the curve of GA/HbA1c for diagnosis of cognitive dysfunction was 0.669(95%CI:0.575-0.763,P<0.05).Conclusion GA/HbA1c is related to cognitive dysfunction and has a certain predictive value for cognitive dysfunction in older patients with T2DM.
