温阳利水方治疗慢性肺源性心脏病临床效果及对患者心脏功能、免疫功能影响研究

摘　　要：目的观察温阳利水方治疗慢性肺源性心脏病临床效果及对心脏功能、免疫功能影响。方法将医院肺病科收治的109例慢性肺源性心脏病患者(2019年6月—2022年6月收治)纳入实验,以随机数字法将纳入患者分成参照组(54例)与研究组(55例)两组,参照组患者均接受西医常规治疗,研究组患者在参照组治疗基础上结合温阳利水方治疗,两组数据比较:疗效、治疗前后心脏功能指标与肺功能指标、中医证候积分(全身浮肿、心悸气短、形寒肢冷、四肢沉重、小便不利等)变化、免疫功能提升率、治疗前后患者6 min步行距离与生活质量评分变化、治疗不良反应。结果研究组与参照组治疗总有效率比较(96.36%vs 85.19%),研究组更高(P<0.05);各组患者治疗前肺动脉压与右心室内径(right ventricular internal diameter,RVID)、左心室内径(left ventricular internal dimension,LVID)等心脏功能指标、最大通气量(maximal voluntary ventilarion,MVV)与第1秒用力呼气容量(forced expiratory volume in one second,FEV_(1))、FEV_(1)/用力肺活量(forced vital capacity,FVC)等肺功能指标、中医证候积分(全身浮肿、心悸气短、形寒肢冷、四肢沉重、小便不利等)、6 min步行距离、生活质量评分等指标比较,P>0.05,各组患者治疗后肺动脉压、RVID及LVID、MVV、FEV_(1)、FEV_(1)/FVC、中医证候积分(全身浮肿、心悸气短、形寒肢冷、四肢沉重、小便不利等)、6 min步行距离、生活质量评分等指标均改善,研究组患者治疗后肺动脉压、RVID及LVID、MVV、FEV_(1)、FEV_(1)/FVC、中医证候积分(全身浮肿、心悸气短、形寒肢冷、四肢沉重、小便不利等)、6 min步行距离、生活质量评分等指标均优于参照组患者(P<0.05);研究组免疫功能提升率(92.59%)比参照组(62.96%)更高(P<0.05);两组均未见不良反应。结论温阳利水方治疗慢性肺源性心脏病临床效果显著,患者症状显著好转,心脏功能Objective To observe the clinical effect of Wenyang Lishui Decoction(温阳利水方)on chronic pulmonary heart disease and its influence on cardiac function and immune function.Methods A total of 109 patients with chronic pulmonary heart disease admitted to the pulmonary diseases department of the hospital from June 2019 to June 2022 were included in this experiment.The patients were divided into two groups by random number method:reference group(54 cases)and study group(55 cases).The patients in the reference group received conventional western medicine treatment(cefoperazone sulbactam,ciprofloxacin and levofloxacin,aminophylline,ambroxol hydrochloride,tovalptan,etc.).Those in the study group were treated with Wenyang Lishui Decoction on the basis of the treatment in the reference group.The efficacy,changes in cardiac function indexes and lung function indexes before and after treatment,changes in TCM syndrome scores,improvement rate of immune function,changes in 6-minute walking distance and quality of life score before and after treatment and adverse reactions of two groups were compared.Results The total effective rate was higher in the study group(96.36%vs 85.19%)(P<0.05).Before treatment,pulmonary artery pressure,right ventricular internal diameter(RVID),left ventricular internal dimension(LVID)and other cardiac function indexes,maximum ventilation(MVV),forced expiratory volume in one second(FEV_(1)),FEV_(1)/forced vital capacity(FVC)and other pulmonary function indexes of patients in each group,comparison of TCM syndrome score(edema,palpitations,shortness of breath,cold limbs,heavy limbs,poor urination,etc.),6-minute walking distance,quality of life score and other indicators were not significant(P>0.05).After treatment,pulmonary artery pressure,RVID,LVID,MVV,FEV_(1),FEV_(1)/FVC,TCM syndrome scores(edema,palpitations,shortness of breath,cold limbs,heavy limbs,poor urination,etc.),6-minute walking distance,the quality of life score and other indicators were improved.After treatment,the pulmonary artery pre
