摘  要:目的观察二苯乙烯苷(TSG)对糖尿病(DM)大鼠肾组织血管内皮生长因子(VEGF)表达干预,探讨TSG延缓糖尿病肾病(DN)大鼠肾小球滤过率下降机理。方法雄性Wister大鼠150只,分正常组(NC组)48只;余102只高糖高脂喂养,8周后造DM大鼠模型。其中95只大鼠造模成功,随机分为模型组(DN组)50只;二苯乙烯苷治疗组(DN+TSG组)45只,TSG(30 mg·kg^(-1)·d^(-1))腹腔注射干预8周,实验观察共20周。HE染色、光镜、电子显微镜下观察各组大鼠肾脏组织形态学的变化;免疫组织化学染色方法测定大鼠肾组织中VEGF的表达变化。结果TSG干预8周时及给药结束后即实验12、16、20周时,DN组与NC组比较:尿微量白蛋白/肌酐(UALB/Ucr)、总胆固醇(CHOL)、血肌酐(Scr)逐渐升高(P<0.05)。DN+TSG组较DN组:UALB/Ucr、CHOL、Scr降低(P<0.05)、病理变化减轻、VEGF在肾组织中表达减少(P<0.05)。结论何首乌提取物TSG延缓DN大鼠肾功能不全进展,可能与其减轻DN大鼠肾脏VEGF表达,减轻氧化应激对肾脏血管内皮损伤,减少蛋白尿有关。Objective To observe the intervention of stilbene glycoside(TSG)on the expression of vascular endothelial growth factor(VEGF)in renal tissue of diabetes rats and explore the mechanism of TSG delaying the decline of glomerular filtration rate in diabetes nephropathy(DN)rats.Methods A total of 150 male Wister rats were randomly divided into two groups:42 normal controls(NC group)and the rest 102 rats with high glucose and high fat for 8 weeks were injected STZ to establish diabetic nephropathy rat model.Ninety-five rats of the rest 105 rats established successfully were also randomly divided into two groups:50 diabetic nephropathy(DN group)and 45 diabetic nephropathy were intraperitoneal injected TSG for 8 weeks(DN+TSG group),and then it was observed for 20 weeks.Morphological changes of kidney tissues in each group were observed by HE staining and electron microscopy.The expression of VEGF in rat kidney was determined by immunohistochemical staining.Results At the 8th week of TSG intervention and the 12th,16th and 20th week of the experiment after the end of administration,the urinary microalbumin/creatinine(UALB/Ucr),total cholesterol(CHOL)and serum creatinine(Scr)in DN group were gradually increased compared with those of the NC group(P<0.05).Compared with the DN group,DN+TSG group showed lower levels of UALB/Ucr,CHOL and Scr(P<0.05),less pathological changes and lower expression of VEGF in renal tissue(P<0.05).Conclusion TSG,the extract of Heshouwu(Polygonum multiflorum Thunb.),delays the progress of renal function in DN rats,which may be related to the reduction of VEGF expression in the kidney of DN rats,the reduction of damage to renal vascular endothelium caused by oxidative stress and the reduction of proteinuria.
