光子治疗仪治疗手外伤穿支皮瓣修复术后感染的临床效果

摘　　要：目的探讨光子治疗仪治疗手外伤组织缺损伤口感染的效果,为手外伤的治疗提供技术参考。方法选取2020年01月-2021年12月于滨州医学院附属医院因手外伤组织缺损行皮瓣修复术的96例患者,按数字表法随机分为研究组48例和对照组48例。术后均予以常规换药处理,研究组另额外予以光子治疗仪治疗。比较两组患者抗菌药物使用时间、住院时间、皮瓣成活率、创面愈合时间、创面瘢痕评分及手功能。结果研究组抗菌药物使用时间和住院时间均明显短于对照组(P<0.05),但两组皮瓣成活率比较差异无统计学意义。研究组患者创面愈合时间(12.20±2.86)d及温哥华瘢痕量表(VSS)评分(6.21±1.10)均明显低于对照组,差异具有统计学意义(P<0.05)。治疗后30 d研究组优良率为95.83%,对照组为64.58%,研究组优良率明显高于对照组(χ^(2)=9.560,P=0.002)。结论光子治疗仪治疗可减少手外伤组织缺损伤患者抗菌药物使用时间、住院时间及伤口愈合时间,抑制瘢痕增生并改善手部功能,值得在临床上推广应用。OBJECTIVE To observe the effect of photon therapy instrument on treatment of the hand trauma tissue defect patients with wound infection so as to provide technical reference for treatment of hand trauma.METHODS A total of 96 patients who underwent free flap repair due to hand trauma tissue defect in Binzhou Medical University Hospital from Jan 2020 to Dec 2021 were enrolled in the study and randomly divided into the study group with 48 cases and the control group with 48 cases.All of the patients were given routine dressing change after the surgery,while the study group was given additional photon therapy.The time of use of antibiotics,length of hospital stay,survival rate of flap,wound healing time,score of wound scar and hand function were observed and compared between the two groups of patients.RESULTS The time of use of antibiotics and length of hospital stay were remarkably shorter in the study group than in the control group(P<0.05),however,there was no significant difference in the survival rate of flap between the two groups.The wound healing time of the study group was(12.20±2.86)days,remarkably shorter than that of the control group;the Vancouver Scar Scale(VSS)score of the study group was(6.21±1.10)points,remarkably lower than that of the control group;there were significant differences(P<0.05).The excellent and good rate of the study group was 95.83%after the treatment for 30 days,remarkably higher than 64.58%of the control group(χ^(2)=9.560,P=0.002).CONCLUSION The photon therapy may shorten the time of use of antibiotics,length of hospital stay and wound healing time of the patients with hand trauma tissue defect,inhibit the scar hyperplasia and improve the hand function,and it is worthy to be promoted in hospitals.
