基于“脉络-血管系统”探讨络虚通补法在冠心病心绞痛气虚血瘀证中应用

摘　　要：“脉络-血管系统”概念是吴以岭院士在络病理论研究过程中提出的,此概念指出了中医之脉、脉络、孙络与西医学讲述的大、中、小血管及微血管在解剖结构和微观功能方面的高度相似性,而现在认为冠心病心绞痛常涉及冠状动脉之大、中、小及微血管。因此,冠心病心绞痛该归属“心络病”范畴,根据其易滞易瘀、易入难出、易积成形的病机特点,多遵循“络以通为用”的治疗原则。该文将围绕“脉络-血管系统”概念探讨冠心病心绞痛气虚血瘀证的发生、发展特点及证治规律,同时结合临床实践过程中典型案例论证了其治疗方面“以通为补,通补并用”新思维的应用,以及通补并用使气、血、津、液渗濡灌注络脉,心之气络充盈且通畅,冠状动脉之大小循环功能方可逐渐恢复的重要治疗意义,为进一步提高疗效、改善预后提供新的临床思路。“Venation-vascular system”is raised in the research process of the collaterals disease theory by WU Yiling,one of the academicians of Chinese Academy of Engineering.The concept is pointed out the high similarity in anatomical structure and microscopic features between pulse,meridian,minute collateral of TCM and large,medium and small blood vessels and capillaries of western medicine.Coronary heart disease angina belongs to the category of“heart collateral disease”because it involves coronary artery of large,medium and small and capillaries.According to its pathogenesis characteristics of easy stagnation and stasis,easy to enter and difficult to come out,easy to accumulate,it often follow the treatment principle of“venation runs based on dredging”.Therefore,the paper will revolve around“venation-vascular system”concept to explore the occurrence and development characteristics and rule of Qi deficiency and blood stasis type of coronary heart disease angina.At the same time,to demonstrate the application of the new thinking of“taking the dredging as the supplementing and combining both of them”in clinical practice,as well as its important therapeutic significance to combine Qi,blood and fluid infiltration into the collaterals and fill the Qi of heart collateral,which can make the coronary circulation function recovered and provide a new clinical idea for further improving the curative effect and prognosis of such patients.
