糖尿病肾病中医病因病机学说基础探讨

摘　　要：糖尿病肾病为糖尿病的主要临床并发症之一,为典型的微血管病变。该文结合古代医家的相关文献资料,对糖尿病肾病的中医理论框架进行系统梳理,从病因、病机两大方面对其进行追溯与探究。经过查阅大量文献并整理,将糖尿病肾病病因大致归纳为禀赋缺失,脏腑柔弱;数食肥甘,嗜酒成性;情志忧郁,化火伤阴;恣情纵欲,房劳过度;久服丹石,燥热内生;外感六淫,毒邪内侵;病机可归纳为瘀血阻络为其病机之标,脾肾亏虚为其病机之本。研究旨在为临床医疗工作者深入探析糖尿病肾病提供一定的思路和理论指导作用。Diabetic nephropathy is one of the main clinical complications of diabetes,which is a typical microvascular disease.Based on the relevant literature of ancient physicians,this paper systematically sorts out the theoretical framework of traditional Chinese medicine for diabetic nephropathy,and traces and explores it from the two aspects of etiology and pathogenesis.After reviewing a large number of literatures and sorting out,the causes of diabetic nephropathy can be roughly summarized as:lack of endowment,weak viscera;a few fatty foods,alcoholism;emotional depression,turning fire and hurting yin;unbridled sexual desires,excessive housework;prolonged taking medicinal powder,dryness and heat are endogenous;exogenous six evils,poisonous evil invading;the pathogenesis can be summarized as:blood stasis blocking collaterals is the target of the pathogenesis,and deficiency of the spleen and kidney is the root of the pathogenesis.The purpose of this study is to provide some ideas and theoretical guidance for clinical medical workers to deeply analyze diabetic nephropathy.
