摘  要:目的分析苏州市二级及以上医疗机构医院感染现状,为各级医院的医院感染管理和防控工作提供数据支持。方法通过市区域性医院感染监测平台系统提取参与监测医院2020年1月-12月医院感染管理质量控制相关监测指标,对二级医院和三级医院的相关数据进行统计分析。结果2020年41所医院1324890例住院患者中,新发医院感染病例18353例,医院感染发病率为1.39%,医院感染病例漏报率为6.95%,I类切口手术部位感染率为0.30%,I类切口手术抗菌药物预防使用率为17.58%,血管内导管相关血流感染发病率为0.343‰,呼吸机相关肺炎发病率为6.528‰,导尿管相关泌尿系感染发病率为1.176‰。不同级别医疗机构I类切口手术抗菌药物预防使用率无统计学差异,其他各项医院感染管理质量控制相关指标三级医院均高于二级医院(P<0.05)。结论本市不同级别医疗机构医院感染管理质量控制监测指标存在差异,各医院应参考监测结果并结合实际情况制定有效的监测策略和防控方案,降低感染发生的风险。OBJECTIVE To investigate the current status of nosocomial infection in secondary or above medical institutions of Suzhou so as to provide data support for management,prevention and control of nosocomial infection in hospitals at all levels.METHODS The related monitoring indexes for nosocomial infection management quality control that the hospitals participated from Jan 2020 to Dec 2020 were extracted by regional nosocomial infection monitoring platform.The related data from the secondary and tertiary hospitals were statistically analyzed.RESULTS Among 1324890 hospitalized patients from the 41 hospitals in 2020,18353 were new onset cases of nosocomial infection,with the incidence of nosocomial infection 1.39%;the missing report rate of nosocomial infection cases was 6.95%;the incidence rate of type I incision site infection was 0.30%,the rate of antibiotics prophylaxis was 17.58%for the type I incision surgery;the incidence of endovascular catheter-related bloodstream infection was 0.343‰,the incidence of ventilator-associated pneumonia was 6.528‰,and the incidence of urinary catheter-related urinary tract infection was 1.176‰.There was no significant difference in the rate of antibiotics prophylaxis for the type I incision surgery among the medical institutions at different levels,the rest of the related indexes for nosocomial infection management quality control were higher in the tertiary hospitals than in the secondary hospitals(P<0.05).CONCLUSION There are significant differences in the monitoring indexes for nosocomial infection management quality control among the different levels of medical institutions in the city.It is necessary for the hospitals to formulate effective monitoring strategies and prevention programs based on the monitoring results so as to reduce the risk of infection.
