摘　　要：目的研究在国际医疗卫生机构认证联合委员会(JCI)评审背景下,运用感控工作间APP搭建线上学习、培训平台,观察多因素协同作用下手卫生持续改进效果。方法选取中南大学湘雅医院国际医疗部所有人员为观察对象,从2018年第一季度开始按照JCI认证模式,实施一揽子手卫生干预措施,分析2018年四个季度手卫生依从性变化。结果国际医疗部启动认证准备以来,手卫生依从率从第一季度的55.56%上升到第四季度的85.39%(P<0.05);不同科室及岗位手卫生依从率均有显著提高(P<0.05);按照WHO 5个手卫生指征,除接触患者血液体液后其余四个指征依从性均呈上升趋势(P<0.05);国际医疗部医院感染率从2018年度第一季度的5.91%(11/186)下降至第四季度的1.64%(4/244),趋势χ2检验结果显示感染率呈下降趋势(χ^(2)=7.184,P=0.007)。结论在医院内推广JCI思维,形成促进医疗质量与病人安全持续改进的氛围,借助互联网技术手段,多因素协同作用,可以持续提高医务人员手卫生依从性。OBJECTIVE To set up the online learning and training platforms by Infection Control Center Application under accreditation circumstance of Joint Commission on Accreditation of Healthcare Organizations(JCI)and observe the continuous improvement effect of hand hygiene under synergistic effect of multiple factors.METHODS All of the health care workers of international medical department of Xiangya Hospital,Central South University were recruited as the research subjects.A series of hand hygiene intervention measures were carried out according to the JCI accreditation model from the first quarter of 2018,and the change of hand hygiene compliance in the four quarters of 2018 was observed.RESULTS Since the start of accreditation preparation by the international medical department,the hand hygiene compliance rate was increased from 55.56%in the first quarter to 85.39%in the fourth quarter(P<0.05).The hand hygiene compliance rate of the various departments and positions was remarkably improved(P<0.05).The compliance to all the 5 hand hygiene indications of WHO except for the contact with patient′s blood and body fluid showed upward trend(P<0.05).The incidence of nosocomial infection in the international medical department was decreased from 5.91%(11/186)in the first quarter of 2018 to 1.64%(4/244)in the fourth quarter.The trend chi-square test showed that the infection rate showed a downward trend(χ^(2)=7.184,P=0.007).CONCLUSION It is necessary for the hospital to promote the JCI thinking to create an atmosphere for continuous improvement of medical quality and patient′s safety and continuously improve the hand hygiene compliance of the health care workers with the help of internet technology and synergistic effect of multiple factors.
