摘 要:目的验证Hoffmann法在建立杭州市德清地区0~6岁不同年龄区间儿童全血钙(Ca)、铁(Fe)、锌(Zn)和镁(Mg)参考区间的可靠性和可行性。方法将2019年1月至2021年6月在杭州师范大学附属德清医院体检的5608例0~6岁儿童纳入本次回顾性研究,采用火焰原子吸收光谱(AAS)法测定儿童全血中的Ca、Fe、Zn和Mg。剔除离群值后,用Hoffmann法建立不同年龄区间儿童全血Ca、Fe、Zn和Mg参考区间,然后与厂家推荐的参考区间相比较,然后收集2021年7月至2022年6月在杭州师范大学附属德清医院体检的1106例健康儿童(0~6岁)全血Ca、Fe、Zn和Mg数据验证Hoffmann法建立的各参考区间的可行性。结果基于Hoffmann法建立的0~1岁全血Ca、Fe、Zn和Mg参考区间分别是(1.73~2.40)mmol/L、(6.40~8.65)mmol/L、(35.50~84.56)μmol/L和(1.35~2.07)mmol/L;1~2岁分别为(1.66~2.34)mmol/L、(6.48~8.82)mmol/L、(38.32~86.77)μmol/L和(1.31~1.98)mmol/L;2~3岁分别为(1.58~2.29)mmol/L、(6.67~8.97)mmol/L、(46.61~90.90)μmol/L和(1.27~1.92)mmol/L;3~4岁分别为(1.51~2.23)mmol/L、(6.82~9.13)mmol/L、(51.83~96.12)μmol/L和(1.23~1.88)mmol/L;4~5岁分别为(1.45~2.19)mmol/L、(7.05~9.26)mmol/L、(56.04~100.07)μmol/L和(1.19~1.84)mmol/L;5~6岁分别为(1.40~2.15)mmol/L、(7.16~9.39)mmol/L、(60.11~105.13)μmol/L和(1.15~1.79)mmol/L。基于Hoffmann法建立的各年龄层全血Ca、Fe、Zn和Mg参考区间与厂家提供的参考区间存在一定差异;在1106例0~6岁健康体检儿童中的验证结果显示,6个年龄组(0~1、1~2、2~3、3~4、4~5、5~6岁)超出参考区间上下限界值的总比例[Hoffmann法建立vs.厂家提供]分别为Ca:3.48%vs.7.82%、1.94%vs.6.28%、1.97%vs.5.53%、2.73%vs.5.92%、1.98%vs.4.61%和1.25%vs.5.63%;Fe分别为2.61%vs.7.83%、2.42%vs.7.63%、1.56%vs.6.81%、1.82%vs.6.39%、1.32%vs.6.58%和0.63%vs.3.75%;Zn分别为2.71%vs.6.96%、2.41%vs.5.31%、1.59%vs.5.54%、1.37%vs.4.56%、1.32%vs.4.60%、0.63%vs.1.88%,Mg分别为5.22%vs.6.96%、3.38%vs.5.31%、2.77%vs.4.75%Objective To verify the reliability and feasibility of Hoffmann method in establishing the reference range of whole blood calcium(Ca),iron(Fe),zinc(Zn)and magnesium(Mg)in children aged 0 to 6 years old in Hangzhou Deqing District.Methods A total of 5608 children aged 0-6 years old in the Deqing Hospital Affiliated to Hangzhou Normal University from January 2019 to June 2021were included in this retrospective study.Ca,Fe,Zn and Mg in whole blood of children were determined by flame atomic absorption spectrometry(AAS).After excluding outliers,the reference intervals of Ca,Fe,Zn and Mg in whole blood of children in different age groups were established by Hoffmann method,and then compared with the reference intervals recommended by manufacturers.Then the whole blood Ca,Fe,Zn and Mg data of 1106 healthy children(0-6 years old)recruited in the Deqing Hospital Affiliated to Hangzhou Normal University from July 2021 to June 2022 were collected to verify the feasibility of each reference interval established by Hoffmann method.Results The reference intervals of whole blood Ca,Fe,Zn and Mg established by Hoffmann method in 0~1 years old were(1.73-2.40)mmol/L,(6.40-8.65)mmol/L,(35.50-84.56)μmol/L and(1.35-2.07)mmol/L,respectively.1-2 years old were(1.66-2.34)mmol/L,(6.48-8.82)mmol/L,(38.32-86.77)μmol/L and(1.31-1.98)mmol/L,respectively.2-3 years old were(1.58-2.29)mmol/L,(6.67-8.97)mmol/L,(46.61-90.90)μmol/L and(1.27-1.92)mmol/L,respectively.3-4 years old were(1.51-2.23)mmol/L,(6.82-9.13)mmol/L,(51.83-96.12)μmol/L and(1.23-1.88)mmol/L,respectively.4-5 years old were(1.45-2.19)mmol/L,(7.05-9.26)mmol/L,(56.04-100.07)μmol/L and(1.19-1.84)mmol/L,and 5~6 years old were(1.40-2.15)mmol/L,(7.16-9.39)mmol/L,(60.11-105.13)μmol/L and(1.15-1.79)mmol/L,respectively.There was a certain difference between the reference interval of each index(whole blood Ca,Fe,Zn,Mg)for each age group established based on Hoffmann method and the reference interval provided by the manufacturer.The verification results of 1106 healthy children aged 0-
