摘　　要：目的比较关节镜下囊肿交通瓣膜扩大和关节镜下腘窝囊肿切除治疗胭窝囊肿的效果。方法采用回顾性分析,选取2016年6月至2020年6月聊城市人民医院骨外科收治的腘窝囊肿患者68例作为研究对象。根据手术治疗方法不同分为观察组(n=33)与对照组(n=35),观察组使用关节镜下腘窝囊肿切除术治疗,对照组则使用关节镜下交通瓣膜扩大术治疗。观察两组患者手术一般情况。观察两组患者手术前与手术后3、6和12个月进行随访时视觉模拟评分法(VAS)、Lysholm评分;记录并发症与复发率;采用Rausching和Lindgren膝关节腘窝囊肿分级评分标准进行疗效评估。结果两组患者出血量与手术切口长度比较,差异均无统计学意义(P>0.05);观察组手术时间为(75.21±4.08)min,显著长于对照组[(30.23±4.26)min],但住院时间为(4.59±1.38)d,短于对照组[(6.26±1.41)d],差异均有统计学意义(P<0.05)。两组患者手术前后VAS、Lysholm评分比较,差异均无统计学意义(P>0.05)。两组患者手术后1年随访切口愈合良好,无感染、瘘管和血肿,且均无复发,无血管神经损伤。两组手术前后Rausching和Lindgren分级结果差异有统计学意义(P<0.05),两组之间手术前、手术后Rausching和Lindgren分级结果差异无统计学意义(P>0.05)。结论关节镜下单独内引流治疗腘窝囊肿临床疗效显著,能有效促进患者术后恢复,降低手术时间、且能促进膝关节功能改善和降低术后复发率。Objective To compare the clinical efficacy of arthroscopic communication enlargement and arthroscopic excision in the management of popliteal cysts.Methods A retrospective analysis was conducted on 68 patients with popliteal cyst admitted to the Department of Orthopedics,Liaocheng People's Hospital from June 2016 to June 2020.Patients were classified into two groups according to the different surgical treatment methods.The observation group(n=33)received arthroscopic excision of popliteal cysts,while control group(n=35)received arthroscopic communication enlargement.The operation related indexes were recorded in both groups.The visual analogue scale(VAS)and Lysholm score were observed before and at 3,6 and 12 months after operation.The complication rate and recurrence rate were compared between groups,and the functional outcome was evaluated by use of the Rauschning and Lindgren knee score.Results There was no statistically significant difference between the amount of bleeding and the length of surgical incision between the two groups(P>0.05);the operation time of the observation group was(75.21±4.08)min,which was significantly longer than that of the control group[(30.23±4.26)min],but the hospitalization time was(4.59±1.38)d,which was shorter than that of the control group[(6.26±1.41)d],the differences were statistically significant(P<0.05).There was no statistically significant difference in VAS and Lysholm scores between the two groups before and after operation(P>0.05).The 1-year postoperative follow-up showed good healing in both groups with no infection,fistulas,haematoma,recurrence,and vascular or nerve damage in either group.There was statistically significant difference in Rausching and Lindgren grading results before and after operation between the two groups(P<0.05),but there was no statistically significant difference in Rausching and Lindgren grading results before and after surgery between the two groups(P>0.05).Conclusion The clinical outcomes of arthroscopic internal drainage alone for poplite
