老年慢性病出院患者对居家护理服务的需求现状调查及影响因素分析

摘　　要：目的通过对三级公立医院联合社区护理站开展居家护理服务试点工作的随访调查,了解老年慢性病出院患者对居家护理服务的需求现状及影响因素,为我国居家护理服务模式的建立和运行等提供参考。方法选取2020年8月至2021年12月成都市温江区人民医院和社区护理站60岁以上的慢性病出院患者260名,由调查人员采用问卷星或问询式调查法进行“一对一”随访调查。统计老年慢性病出院患者的一般资料;分析老年慢性病出院患者对居家护理服务的需求现状;采用Logistic回归分析老年慢性病出院患者对居家护理服务需求的影响因素。结果愿意选择居家护理服务的人数占60.4%,其中73.2%的人通过医院医护人员宣传了解该服务;“制定健康管理计划并进行随访”“生命体征监测”是排名前2位的项目,分别占51.0%、33.8%;老年人不愿选择居家护理服务的最主要原因是对居家护理服务不够了解,占比为41.7%。单因素分析结果显示,有居家护理服务需求与无居家护理服务需求患者的文化程度、月收入、日常生活能力评分、认知能力评分比较,差异具有统计学意义(P<0.05)。Logistic回归分析结果显示,文化程度、月收入、日常生活能力评分、认知能力评分是老年慢性病出院患者居家护理服务需求的独立影响因素(P<0.05)。结论三级公立医院联合社区护理站的居家护理模式能有效将医院优质的医疗护理服务进行延伸,但老年慢性病患者对该类服务的知晓度、需求度不高,在未来的工作中相关人员应加强宣传、及时了解市场需求,以需求为导向增加服务项目、提升服务质量,重点关注高龄老年人、失能失智人群的整体健康水平和生存质量,进一步完善老年健康保障机制,促进实现健康老龄化。Objective To investigate the current demand and influencing factors for home care services in elderly discharged patients with chronic diseases through a follow-up survey on the pilot work of home care services carried out by tertiary public hospitals and community nursing stations,and to provide references for the establishment and operation of home care service models in China.Methods From August 2020 to December 2021,260 discharged patients with chronic diseases over 60 years old from Wenjiang District People's Hospital of Chengdu and community nursing station were selected,and"one-to-one"follow-up survey was conducted by investigators using questionnaire stars or inquiry survey method.The general data of elderly discharged patients with chronic diseases were collected;the demand status for home care services in elderly discharged patients with chronic diseases was analyzed;Logistic regression was used to analyze the influencing factors of home care services of elderly discharged patients with chronic diseases.Results The number of patients who were willing to choose home care services accounted for 60.4%,of which 73.2%were aware of the service through hospital medical staff publicity;the top two items were"health management plan and follow-up"and"vital signs monitoring",accounting for 51.0%and 33.8%respectively;the main reason why the elderly are unwilling to choose home care services is that they do not know enough about home care services,accounting for 41.7%.The univariate analysis results showed that there were statistically significant differences in educational level,monthly income,daily living ability score and cognitive ability score between patients with home care service needs and without home care service needs(P<0.05).The results of Logistic regression analysis showed that educational levels,monthly income,daily living ability score and cognitive ability score were independent influencing factors of home care services of elderly discharged patients with chronic diseases(P<0.05).Conclusion The home
