基于扎根理论的护理干预结合早期体位活动指导对胆囊切除术后患者胃肠蠕动功能、睡眠质量及心理弹性的影响

摘　　要：目的探讨基于扎根理论的护理干预结合早期体位活动指导在胆囊切除术后的应用效果。方法将2018年2月至2021年2月收治的80例胆囊切除术后患者采用随机数字表法分为对照组(40例,常规术后护理)和观察组(40例,在对照组基础上实施基于扎根理论的护理干预结合早期体位活动指导)。比较两组的干预效果。结果干预后,观察组的胃泌素(GAS)、胃动素(MTL)水平高于对照组,血管活性肠肽(VIP)水平低于对照组(P<0.05)。干预后,观察组的匹兹堡睡眠质量指数(PSQI)各维度评分均低于对照组(P<0.05)。干预后,观察组的自强、坚韧、乐观评分高于对照组(P<0.05)。结论基于扎根理论的护理干预结合早期体位活动指导能促进胆囊切除术后患者胃肠蠕动,改善胃肠激素水平,也能提高其睡眠质量及心理弹性。Objective To explore the application effect of nursing intervention based on grounded theory combined with early postural activity guidance after cholecystectomy.Methods From February 2018 to February 2021,80 patients after cholecystectomy were divided into control group(40 cases,routine postoperative nursing)and observation group(40 cases,nursing intervention based on grounded theory combined with early postural activity guidance on the basis of the control group)by random number table method.The intervention effects of the two groups were compared.Results After intervention,the levels of gastrin(GAS)and motilin(MTL)in the observation group were higher than those in the control group,the level of vasoactive intestinal peptide(VIP)was lower than that in the control group(P<0.05).After intervention,the scores of each dimension of Pittsburgh Sleep Quality Index(PSQI)in the observation group were lower than those in the control group(P<0.05).After intervention,the scores of self-improvement,tenacity and optimism in the observation group were higher than those in the control group(P<0.05).Conclusion Nursing intervention based on grounded theory combined with early postural activity guidance can promote gastrointestinal peristalsis,improve gastrointestinal hormones levels,and improve sleep quality and psychological resilience of patients after cholecystectomy.
