摘 要:目的观察无针注射器注射胰岛素对糖尿病(DM)患者注射部位疼痛、局部皮肤反应和糖化血红蛋白(HbA1c)的影响。方法选择2021年3—9月接诊且行胰岛素治疗的60例DM患者,随机分为观察组(30例,以无针注射器给药)、对照组(30例,以胰岛素笔给药),评价2组血糖相关指标、不同时段HbA1c、局部皮肤反应情况、中国胰岛素治疗满意度量表中文版(ITSQ)评分、低血糖发生率。结果治疗前,组间血糖相关指标[C肽、空腹胰岛素(FINS)、空腹血糖(FPG)、餐后2 h血糖(2 hPG)]无明显差异(P>0.05);治疗后,观察组C肽、FINS、FPG、2 hPG均优于对照组(P<0.05)。治疗前,组间HbA_(1)c无明显差异(P>0.05);观察组治疗70 d的HbA_(1)c低于对照组(P<0.05)。较之对照组,观察组局部皮肤反应、低血糖发生率更低,且ITSQ评分更高(P<0.05)。结论对DM患者以无针注射器注射胰岛素,能够增强血糖控制效果,减少局部皮肤反应,提高治疗满意度,值得推广。Objective To observe the effect of needle-free injection of insulin on injection site pain,hematoma induration outcome and glycosylated hemoglobin(HbAjc)in patients with diabetes(DM).Methods 60 patients with DM who were treated with insulin from March 2021 to September 2021 were randomly divided into observation group(30 cases were included and administered with needle less syringe)and control group(30 cases were included and administered with insulin pen).The blood glucose related indexes,HbAic in different periods,local skin reactions,Chinese version of Chinese insulin treatment satisfaction scale(itsq)Incidence of hypoglycemia.Results There was no significant difference in blood glucose related indexes[C-peptide,fasting insulin(fins),fasting blood glucose(FPG),2h postprandial blood glucose(2hPG)]between the groups before treatment(P>0.05);After treatment,C-peptide,fins,FPG and 2hPG were better in the observation group(P<0.05).Before treatment,there was no significant diference in HbA_(1)c between the groups(P>0.05);HbA_(1)c in the observation group was lower at 70 days of treatment(P<0.05).Compared with the control group,the incidence of local skin reaction and hypoglycemia in the observation group was lower,and the itsq score was higher(P<0.05).Conclusion Insulin injection with needle less syringe in patients with DM can enhance the effect of blood glucose control,reduce local skin reaction and improve treatment satisfaction,which is worthy of popularization.
