超声测量IVC评估重度子痫前期患者术后血容量的变化

摘　　要：目的床旁超声测量下腔静脉内径(IVC)评估重度子痫前期患者剖宫产术后血容量变化趋势。方法选取2019年3月至2021年6月本院收治的重度子痫前期产妇40例为观察对象,在麻醉前、手术结束时、术后每6 h(至术后72 h)共14个时间点,使用床旁超声仪选用M模式采集约10 s IVC数据,记录每个时间点呼气相下腔静脉最大内径(IVCe)及吸气相下腔静脉最小内径(IVCi),并计算出IVC随呼吸变化率(ΔIVC)。结果不同时间点的IVCe数据差异有统计学意义(P<0.001),最低点在术后即刻,最高点在术后12 h;不同时间点的ΔIVC数据差异无统计学意义(P=0.24)。结论重度子痫前期患者剖宫产术后即刻是血容量相对较低时间点,术后12 h是血容量相对较高时间点。Objective To evaluate the change trend of blood volume in patients with severe preeclampsia after cesarean section by measuring the diameter of inferior vena cava(IVC)by bedside ultrasound.Methods Forty pregnant women with severe preeclampsia in our hospital were selected as the research objects.At fourteen time points[before anesthesia,at the end of operation and every 6 hours after operation(to 72 hours after operation)],the M mode was used to collect about 10 s internal diameter data of IVC by bedside ultrasound.The maximum internal diameter of IVC in expiratory(IVCe)and the minimum diameter of IVC in inspiratory(IVCi)at each time point were recorded,and the change rate of IVC diameter(ΔIVC)with respiration was calculated.Results There was significant difference in IVCe at different time points(P<0.001).The lowest point was immediately after operation and the highest point was 12 hours after operation.There was no significant difference inΔIVC at different time points(P=0.24).Conclusion The blood volume of patients with severe preeclampsia is relatively low immediately after cesarean section,and the blood volume is relatively high 12 hours after cesarean section.
