摘 要:目的比较三种检测方法(病毒分离培养鉴定法、核酸序列测定法和PCR-荧光探针法)检测鼻咽拭子标本中鼻病毒(HRV)的可靠性。方法采用盲法、对比试验设计。根据纳入标准和排除标准入组220例鼻咽拭子标本,分别用病毒分离培养鉴定法、核酸序列测定法和PCR-荧光探针法进行盲法检测,以评价三种检测方法的临床应用性能。结果本次试验入组的220例鼻咽拭子样本中,病毒分离培养鉴定法检测结果为阳性的样本20例,核酸序列测定法检测结果为阳性的样本32例,PCR-荧光探针法检测结果为阳性的样本35例。与病毒分离培养相比,PCR-荧光探针法检测阳性符合率为100.00%,阴性符合率为92.50%,总符合率为93.18%;PCR-荧光探针法与病毒分离培养鉴定法检测结果一致性一般(kappa=0.692),与核酸序列测定法检测结果一致性较好(kappa=0.947)。结论PCR-荧光探针法检测鼻咽拭子标本中的HRV可靠、准确、安全、简便、稳定,具有较高的临床应用价值。Objective To compare the reliability of three detection methods(virus isolation and culture identification method,nucleic acid sequencing method and PCR fluorescent probe method)in detecting rhinovirus in nasopharyngeal swab samples.Methods A blind and comparative experimental design was used.According to the inclusion criteria and exclusion criteria,220 nasopharyngeal swab samples were detected by virus isolation and culture identification method,nucleic acid sequencing method and PCR fluorescent probe method,respectively,so as to evaluate the clinical application performance of the three detection methods.Results Among the 220 nasopharyngeal swab samples enrolled in this test,20 samples were positive by virus isolation and culture identification method,32 samples were positive by nucleic acid sequencing method and 35 samples were positive by PCR fluorescent probe method.Compared with virus isolation and culture,the positive coincidence rate of PCR fluorescent probe was 100.00%,the negative coincidence rate was 92.50%,and the total coincidence rate was 93.18%.The results of PCR fluorescent probe method and virus isolation and culture identification method were generally consistent(kappa=0.692),and the results of PCR fluorescent probe method and nucleic acid sequencing method were in good agreement(kappa=0.947).Conclusion PCR fluorescence probe method is reliable,accurate,safe,simple and stable for the detection of rhinovirus in nasopharyngeal swabs.
