摘  要:目的 分析三血管二维连续横切面联合多普勒血流及时间-空间相关成像(STIC)技术对先天性血管环的产前诊断价值。方法 选取接受产前超声心动图检查的1 521例胎儿为研究对象,均采用三血管二维连续横切面联合多普勒血流、多普勒血流联合STIC技术诊断胎儿先天性血管环,并随访产后检查结果。结果 产后随访显示24例为先天性血管环胎儿,其中迷走右锁骨下动脉、右位主动脉弓伴迷走左锁骨下动脉为主要类型。24例先天性血管环患儿中,三血管二维连续横切面联合多普勒血流明确诊断为先天性血管环22例,多普勒血流联合STIC技术明确诊断为先天性血管环23例。三血管二维连续横切面联合多普勒血流、多普勒血流联合STIC技术及三者联合诊断胎儿先天性血管环的灵敏度分别为91.67%、95.83%、100.00%,特异度分别为99.80%、99.93%、100.00%。结论 STIC技术诊断胎儿先天性血管环可弥补传统超声心动图对细小血管分辨功能、立体空间感不足的缺点,胎儿三血管二维连续横切面联合多普勒血流及STIC技术对先天性血管环诊断具有较高灵敏度及特异度,具有临床推广价值。Objective To analyze the prenatal diagnosis value of fetal three-vessel two-dimensional continuous cross-section combined with Doppler flow and spatio-temporal image correlation correlation(STIC)technology for congenital vascular rings.Methods A total of 1521 fetuses who received prenatal echocardiography in the hospital were selected as the research objects.In all of them,three-vessel two-dimensional continuous cross-sections combined with Doppler flow,Doppler flow combined with STIC technology were used to diagnose fetal congenital vascular rings.The results of postpartum examination were followed up.Results Postpartum follow-up showed that 24 fetuses were fetuses with congenital vascular ring,among which vagus right subclavian artery,right aortic arch with vagus left subclavian artery were the main types.Among the 24 children with congenital vascular ring,22 children were diagnosed with congenital vascular ring by three-vessel two-dimensional continuous cross section combined with Doppler flow,and 23 children were diagnosed as congenital vascular ring by Doppler flow combined with STIC technology Doppler flow.The sensitivity of three-vessel two-dimensional continuous cross-section combined with Doppler flow,Doppler flow combined with STIC technology,and the combination of the two in the diagnosis of fetal congenital vascular rings were 91.67%,95.83%,and 100.00%,and the specificity were 99.80%,99.93%,and 100.00%,respectively.Conclusions In the diagnosis of fetal congenital blood vessel rings,STIC technology can make up for the shortcomings of traditional echocardiography in the resolution of small blood vessels and the lack of three-dimensional sense.Fetal three-vessel two-dimensional continuous cross-section combined with Doppler flow and STIC technology has high sensitivity and specificity for congenital vascular rings,which has the value of clinical promotion.
