摘　　要：目的探讨左冠状动脉起源异常(AOLCA)的超声心动图特征及术后左心功能改变。方法分析13例AOLCA患儿的超声图像特点,利用二维斑点追踪成像(2D-STI)技术评估外科手术治疗效果。结果13例AOLCA中,左冠状动脉异常起源于肺动脉(ALCAPA)11例,异常起源于右冠窦(AAOLC)2例。首诊超声检出9例,复诊后检出4例。外科手术9例,术后死亡1例,8例存活患儿手术前后左心室舒张期容积(LVEDV)、左心室收缩功能(LVEF)差异显著(P<0.05);2D-STI测量术后左心室整体纵向应变(GLS)、左心室整体轴向应变(GCS)、左心室整体收缩期应变率(GSRs)较术前显著减低,左心室整体扭转度(ROT)较术前显著升高(P<0.05)。结论超声心动图可明确诊断AOLCA并评估术后左心功能改变,对评价外科治疗效果有重要价值。Objective To investigate the echocardiographic characteristics of anomalous origin of left coronary artery(AOLCA)and the changes of left ventricular function after operation.Methods The ultrasound image characteristics of 13 children with AOLCA were analyzed,and the surgical treatmen effect was eraluated by two-dimensional spot tracking imaging(2D-STI).Results Among the 13 cases,ALCAPA in 1l cases and AAOLC in 2 cases.9 cases were detected by ultrasonography at the first visit and 4 cases were detected by ultrasonography at the second visit.There were 9 cases of surgical operation and 1 cases of postoperative death,and 8 cases of survivors had significant differences in LVEDV and LVEF after surgery(P<0.05).2D-STI measure,GLS,GCS and GSRs of the left ventricle were significantly lower than pre-operation,while ROT of the left ventricle was significantly higher than pre-operation(P<0.05).Conclusions Echocardiography can clearly diagnose AOLCA and evaluate the changes of left ventricular function after operation,which is great value in evaluating the effect of surgical treatment.
