摘　　要：目的 观察氟比洛芬酯辅助静脉自控镇痛泵(PCA)对骨科手术患者的术后镇痛效果。方法 选取2021年2月至2022年2月期间信阳一五四医院收治的200例骨科手术患者为研究对象,经抽签法将患者分为观察组和对照组,各100例,对照组术后予以常规PCA镇痛,观察组采用氟比洛芬酯辅助PCA镇痛,比较两组术前、术后心率(HR)、平均动脉压(MAP)等体征指标,比较两组术后1、3、5、7 h等不同时间点下的疼痛情况,术后72 h内PCA按压次数、停用时间、首次下床活动时间及住院时间,比较两组术后甲肾上腺素(NE)、皮质醇(Cor)、血管紧张素-Ⅱ(AT-Ⅱ)等应激指标,不良事件发生情况及术后生活质量。结果 两组患者术前HR、MAP比较,差异无统计学意义(P>0.05);术后,观察组的HR、MAP均低于对照组,观察组术后1、3、5、7 h的VAS评分低于对照组,72 h内PCA按压次数、停用时间、首次下床活动时间及住院时间低于对照组,NE、Cor、AT-Ⅱ低于对照组,不良事件发生率7.00%(7/100)低于对照组19.00%(19/100),简明健康生活状况量表(SF-36)各维度评分高于对照组(P<0.05)。结论 氟比洛芬酯辅助PCA在骨科手术患者的镇痛干预中具有较高应用价值,可在加速患者康复进程同时、有效避免术后应激及其他不良事件发生风险,同时还可有效改善患者术后生活质量。【Objective】 To observe the analgesic effect of flurbiprofen axetil assisted intravenous patient-controlled analgesia(PCA) on patients undergoing orthopedic surgery.【Methods】 Two hundred orthopedic surgery patients admitted to our hospital from February 2021 to February 2022 were selected as the research objects.The patients were divided into an observation group and a control group by lottery,with 100 cases in each group.The control group received conventional PCA after surgery.The observation group was treated with flurbiprofen axetil assisted by PCA for analgesia.The preoperative and postoperative HR,MAP and other physical indicators were compared between the two groups,and the pain conditions at different time points such as 1,3,5,and 7 hours after the operation were compared between the two groups,the number of PCA compressions within 72 hours after surgery,the time of stopping,the time of the first off-bed activity,hospital stay,NE,Cor,AT-Ⅱ,the occurrence of adverse events and the quality of life after surgery were compared between the two groups.【Results】 The preoperative HR and MAP of the two groups of patients were similar(P>0.05);postoperatively,the HR and MAP of the observation group,the VAS score at 1,3,5,and 7 hours after the operation,the number of PCA compressions within 72 hours,the stop time,the time of the first off-bed activity,hospital stay,NE,Cor,and AT-II were lower than those of the control group,the incidence of adverse events of the observation group [7.00%(7/100)] was lower than that of the control group [19.00%(19/100)],and the scores of SF-36 were higher than those of the control group(P<0.05).【Conclusion】 Flurbiprofen axetil assisted PCA can significantly enhance the postoperative analgesic effect of patients undergoing orthopaedic surgery,and has positive significance in reducing postoperative stress reaction,promoting physical recovery,reducing the risk of adverse events and improving the quality of life.
