马建教授从气血论治甲状腺结节经验探析

摘　　要：甲状腺结节(TNS)是内分泌系统的常见疾病,初期大多无任何临床表现,少数可致甲状腺功能亢进,或压迫局部、影响外观,当甲状腺结节体积不断增大,压迫气管时,会出现咳嗽、气促,或喉返神经受累出现声音嘶哑,或食管受压出现吞咽困难。马建教授认为,TNS可以归为中医“瘿病”范畴,气血失调贯穿于其发生发展的全过程,应以中医气血理论为基础,强调气滞为本病的病理基础,而瘀血是导致颈部包块的主要致病因素,故治疗常采用疏肝行气、活血化瘀、消瘿散结法。根据多年临床经验,其总结自拟经验方“玄夏消瘿汤”,临床疗效显著。本文总结介绍马建教授的治疗经验,并附医案1则加以佐证,以飨同道。Thyroid nodule is a common disease of the endocrine system.Most of them have no clinical manifestations in the early stage,and a few can cause hyperthyroidism,or compress the local area and affect the appearance.When the size of the thyroid nodule continues to increase and compresses the trachea,coughing,shortness of breath,hoarseness due to recurrent laryngeal nerve injury,or dysphagia due to esophageal compression can be observed.Professor Ma Jian believes that thyroid nodule can be classified into the category of"goiter"in traditional Chinese medicine(TCM),and qi and blood disorders run through the whole process of its occurrence and development.In the analysis of this disease,the theory of qi and blood in TCM should be taken as the basis to highlight that qi stagnation is the pathological basis of this disease and blood stasis is the main pathogenic factor leading to neck mass.In the treatment,the principles of soothing the liver,moving qi,activating blood,resolving stasis,eliminating goiter,and dissipating mass should be followed.According to many years of clinical experience,Professor Ma self-designed an empirical prescription Xuanxia Xiaoying Decoction with remarkable clinical efficacy.This study summarized and introduced the treatment experience of Professor Ma with a medical case attached to provide a reference for clinical colleagues.
