摘　　要：以中医药学为主体,融合现代医学研究成果与技术方法,创新中医药临床诊疗模式,丰富中医药理论体系,是现阶段中医药创新发展的一大重要方向。然而现代医学背景下的中医药传承创新工作还存在中医传统辨治模式与现代疾病临床诊疗难以有机融合、中医宏观辨治思路与现代医学微观理化指标不能精准对接两个关键问题需要解决,据此提出“两个重构”(即重构现代中医诊疗体系与重构现代中医本草体系)理论框架并对其内涵、目的、方法及意义进行系统阐释。认为通过借鉴现代医学疾病认知,继承传统中医审病思维,对现代疾病进行中医分类分期分证,进而可实现现代中医诊疗体系的重构;同时通过吸纳现代药理研究成果,融汇中药传统功效认识,构建方药量效理论框架推动现代中医本草体系的重构。“两个重构”的实现可在继承并发扬中医调态优势的前提下,突破传统辨证论治局限性,提高中医对现代疾病临床诊疗的精准性,是推动中医药传承创新发展的重要路径。Taking traditional Chinese medicine(TCM)as the main body,integrating modern medical research results and technology methods,innovating clinical diagnosis and treatment mode,and enriching the theoretical system are important directions for the innovative development of TCM at present.However,there are still two key problems to be solved in the process of innovative development of TCM under the background of modern medicine.On the one hand,the traditional syndrome differentiation and treatment mode of TCM is difficult to integrate with modern clinical diagnosis and treatment of diseases;on the other hand,the macro syndrome differentiation and treatment in TCM cannot accurately dock with the microscopic physical and chemical indicators of modern medicine.Therefore,in this paper,we innovatively put forward the theoretical framework of“two reconstructions”,i.e.reconstruction of diagnostic and treatment system and materia medica system in modern Chinese medicine,and systematically explain its connotation,purpose,method and significance.We advocate that by learning from the cognition of modern medicine diseases,inheriting the disease examination thinking of TCM,and classifying and staging modern diseases in the way of TCM,the modern TCM diagnosis and treatment system can be reconstructed.At the same time,we also advocate that by absorbing the results of modern pharmacological research,integrating the understanding of the efficacy of Chinese material medica,and constructing the theoretical framework of dose-response relationship in TCM prescription,the reconstruction of modern Chinese herbal medicine system can be completed.On the premise of inheriting and carrying forward the advantages of TCM,the realization of“two reconstruction”can break through the limitations of traditional treatment based on syndrome differentiation and improve the accuracy of clinical diagnosis and treatment of modern diseases,which is an important path to the preservation and innovative development of TCM.
