摘 要:目的探究不同能量激光预处理对牙本质粘接性能及微渗漏的影响。方法选取2021年5月至2022年5月,冠周炎拔除的104颗无龋人第三磨牙,去除牙冠牙釉质后将其制成牙本质片,分为低激光能量组、高激光能量组、酸性处理组和空白对照组,每组26颗牙,每组52片牙本质片。低能量组采用每个脉冲输出能量85mJ、照射时间30 s、光斑大小400μm预处理;高能量组采用每个脉冲输出能量350 mJ、照射时间30 s、光斑大小400μm预处理;酸性处理组采用9.6%氢氟酸酸蚀处理后不进行激光照射,空白对照组不进行任何处理。采用剪切强度测试检测各组牙本质粘接性能,采用微渗漏实验检测各组牙齿镶嵌的密合性。比较不同能量激光预处理对牙本质粘接性能及微渗漏的影响。结果各组预处理后,酸蚀剂处理组牙本质粘接性能(10.0±2.3)MPa、低能量组牙本质粘接性能(12.2±2.9)MPa、高能量组牙本质粘接性能(14.65±3.49)MPa,显著高于空白对照组的(2.32±0.6)MPa,四组间比较差异具有统计学意义(P<0.05);酸蚀剂处理组牙本质微渗漏(547.4±118.6)μm、低能量组预处理组牙本质微渗漏(482.7±105.4)μm、高能量组牙本质微渗漏(413.5±101.8)μm,均显著低于空白对照组的(861.8±122.5)μm,四组间比较差异具有统计学意义(P<0.05);不同能量激光预处理组牙本质界面破坏、内聚破坏、混合破坏断裂面类型比较,差异无统计学意义(P>0.05)。结论高能量激光预处理可增加牙本质粘接强度,提高牙本质与修复体的密合性,降低微渗漏,进而有助于提高牙修复效果。Objective To explore and analyze the effect of laser pretreatment with different energies on dentin bonding properties and microleakage.Methods Totally 104 caries-free human third molars extracted for pericoronitis over the period from May 2021 to May 2022 were selected and made into dentine tablets after removing crown enamel.They were divided into four groups:the low energy group,high energy group,acid treatment group and blank control group,26 teeth in each group.The teeth in the low energy group were pretreated with the laser of an irradiation energy of 85 mJ.The teeth in the high energy group were pretreated with the laser of an irradiation energy of 350 mJ.The teeth in the acid treatment group were etched with 9.6%hydrofluoric acid without laser irradiation,and those in the blank control group were not treated at all.The shear strength test was used to detect the dentine bonding properties,and the microleakage test was used to detect the dental tessellation for each group.The effect of laser pretreatment with different energies on dentin bonding properties and microleakage was compared.Results The adhesive property of the etch group,low energy group and high energy group were(10.0±2.3)MPa,(12.2±2.9)MPa and(14.7±3.5)MPa respectively,all being much higher than that,i.e.,(2.32±0.6)MPa,of the blank control group.The differences among the four groups were statistically significant(P<0.05).The dentin microleakage in the etch group,low energy group and high energy group were(547.4±118.6)μm,(482.7±105.4)μm and(413.5±101.8)μm respectively,all being much lower than that,i.e.,(861.8±122.5)μm,of the blank control group.The differences among the four groups were statistically significant(P<0.05).There was no significant difference in fracture surface types of dentin interface failure,cohesive failure and mixed failure among different energy laser pretreatment groups(P>0.05).Conclusions Laser pretreatment with high energy density will increase the bonding strength of dentin,improve the tightness of dentin and
