摘  要:目的探讨慢性气道疾病患者使用吸入药物装置的技术水平及其影响因素。方法本研究为一项描述性研究,从2021年8月至2022年3月间至广州医科大学附属第一医院呼吸内科门诊就诊的患者中,随机抽取临床诊断为慢性阻塞性肺疾病(COPD)、支气管哮喘或其他慢性气道疾病,且使用吸入药物治疗的患者为研究对象,采用量表进行吸入装置使用情况、疾病及药物认知程度、用药依从性、吸入装置掌握程度的评估,并采用肺功能仪进行肺通气功能检查。结果206例患者纳入分析,用药依从性评分为6.00(4.19,7.00)分,为依从性中等。吸入技术掌握程度评分为8.00(7.00,9.00)分,为吸入技术掌握较好。其中,COPD组和哮喘组评分分别为7.00(6.00,9.00)分和9.00(7.00,10.00)分,χ^(2)检验结果显示哮喘组吸入技术掌握较好者比例高于COPD组(χ^(2)=11.57,P=0.001)。有序logistic回归分析结果显示年龄、吸入药物认知程度、吸入技术指导次数是影响慢性气道疾病患者使用吸入药物装置技术水平的主要因素。结论年龄、吸入药物认知程度、吸入技术指导次数是影响慢性气道疾病患者使用吸入药物装置技术水平的主要因素。Objective To investigate the technical level and influencing factors of using inhalation drug devices in patients with chronic airway diseases.Methods This study was a descriptive study.Patients who were clinically diagnosed with chronic obstructive pulmonary disease(COPD),bronchial asthma,or other chronic airway diseases and treated with inhaled drugs were randomly selected from the respiratory outpatient department of the First Affiliated Hospital of Guangzhou Medical University between August 2021 and March 2022.Rating scales were used to evaluate the using of inhalation devices,the understanding of chronic airway diseases and therapeutic medication,the compliance with medication,and the mastery level of using inhalation devices.Spirometry was performed for each subject.Results A total of 206 patients were included in the study,and the medication compliance score was 6.00(4.19,7.00),indicating medium compliance.The mastery level score of the inhaler technique was 8.00(7.00,9.00),indicating quite good mastery level.The scores of the COPD group and asthma group were 7.00(6.00,9.00)and 9.00(7.00,10.00),respectively.The chi-square test suggested that the proportion of high-level mastery of inhalation technique in the asthma group was significantly higher than that in the COPD group(χ^(2)=11.57,P=0.001).The ordered Logistic regression analysis showed that age,understanding of inhalation drugs,and number of inhalation technical guidance were the main factors affecting the technical level of inhalation drug devices usage in patients with chronic airway diseases.Conclusions Age,understanding of inhalation drugs,and number of inhaler device guidance were the main factors affecting the technical level of inhaler usage in patients with chronic airway diseases.
