益气补血利水汤联合针刺治疗中风后尿潴留的临床研究

摘　　要：目的:探讨益气补血利水汤联合针刺治疗中风后尿潴留的临床效果。方法:将120例中风后尿潴留患者随机分为对照1组、对照2组和联合组,各40例。对照1组以常规治疗+益气补血利水汤治疗,对照2组以常规治疗+针刺治疗,联合组以常规治疗+益气补血利水汤+针刺治疗。3组患者均以14d为1个疗程,连续治疗3个疗程。治疗后比较各组患者临床疗效,比较3组治疗前后的中医症候积分、尿动力学参数,以及治疗期间不良反应发生率。结果:联合组总有效率高于对照1组、对组2组(P<0.05)。治疗后3组中医症候积分均低于治疗前,联合组中医症候积分均低于对照1组、对照2组,差异均有统计学意义(P<0.05)。治疗后,3组最大尿流量、最大逼尿肌压力、最大膀胱容量高于治疗前,残余尿量低于治疗前,联合组最大尿流量、最大逼尿肌压力、最大膀胱容量高于对照1组、对照2组,残余尿量低于对照1组、对照2组,差异均有统计学意义(P<0.05)。治疗期间3组患者不良反应发生率比较差异无统计学意义。结论:益气补血利水汤联合针刺治疗中风后尿潴留有良好的临床疗效,可改善患者临床症状和尿动力学参数,且有较好的治疗安全性,值得临床应用。Objective:To explore the clinical effect of Yiqi Buxue Lishui decoction combined with acupuncture in the treatment of urinary retention after stroke.Methods:Totally,120 cases of urinary retention after stroke were randomly divided into control group 1,control group 2 and combined group,40 cases in each group.The control group 1 was treated with conventional therapy+Yiqi Buxue Lishui decoction,the control group 2 was treated with conventional therapy+acupuncture,and the combined group was treated with conventional therapy+Yiqi Buxue Lishui decoction+acupuncture.The three groups of patients were treated for 3 consecutive courses with 14 days as a course of treatment.The clinical efficacy of each group was compared after treatment,the TCM syndrome score and urodynamic parameters were compared between the three groups before and after treatment,and the incidence of adverse reactions during treatment was compared between groups.Results:The total effective rate of the combined group was higher than that of the control group 1 and the control group 2(P<0.05),with the difference being statistically significant(P<0.05).After treatment,the TCM symptom scores of the three groups were lower than those before treatment,and those of the combined group were lower than those of the control group 1 and control group 2,with the difference being statistically significant(P<0.05).After treatment,the maximum urine flow,maximum detrusor pressure and maximum bladder volume of the three groups were higher than those before treatment,and the residual urine volume was lower than that before treatment.The maximum urine flow,maximum detrusor pressure and maximum bladder volume of the combined group were higher than those of the control group 1 and control group 2,and the residual urine volume was lower than that of the control group 1 and control group 2,with statistical significance(P<0.05).There was no significant difference in the incidence of adverse reactions among the three groups during treatment.Conclusion:Yiqi Buxue Lishui decoctio
