纱条固定定位法在缝合器式包皮环切术中的应用

摘　　要：目的探讨纱条固定定位法在缝合器式包皮环切术治疗包茎和包皮过长中的临床疗效。方法选取2021年2月至2021年6月在本院行包皮环切的182例患者的临床资料,根据治疗方法分为观察组(92例)和对照组(90例)。观察组采用纱条固定定位法行缝合器式包皮环切术;对照组采用常规缝合器式包皮环切术。观察并分析两组的手术时间、术中出血、系带损伤率、系带对合不良率、术后外观满意度。结果观察组和对照组的手术时间[(6.47±0.36)min vs.(5.77±0.46)min]、系带对合不良率[2.2%(2/92)vs.12.2%(11/90)]、术后外观满意程度[97.8%(90/92)vs.88.9%(80/90)]比较,差异均有统计学意义(均P<0.05);而术中出血量[(2.18±0.57)mL vs.(2.29±0.60)mL]、伤口出血需缝合率[3.3%(3/92)vs.5.6%(5/90)]比较,差异均无统计学意义(均P>0.05)。结论纱条固定定位法在缝合器法包皮环切手术中具有系带对合准确性及外观满意度高的优势,同时不增加术中出血量及伤口出血需缝合率,方法简便,临床效果良好,具有临床推广价值。Objective To investigate the clinical effect of gauze fixation positioning method in the treatment of phimosis and redundant prepuce with suture device circumcision.Methods The clinical data of 182 patients who underwent circumcision in our hospital from February 2021 to June 2021 were selected.According to the treatment methods,they were divided into two groups:observation group(92 cases)and control group(90 cases).The observation group was treated with the gause fixed positioning method,and the control group was treated with the conventional suture device circumcision.The operation time,intraoperative bleeding,frenulum injury rate,frenulum malalignment rate,and postoperative appearance satisfaction of the two groups were observed and analyzed.Results The operation time of the observation group and the control group was[(6.47±0.36)min vs.(5.77±0.46)min],lace-up involution defective rate[2.2%(2/92)vs.12.2%(11/90)],postoperative appearance satisfaction[97.8%(90/92)vs.88.9%(80/90)],the differences were statistically significant(P<0.05).There was no significant difference in intraoperative blood loss[(2.18±0.57)mL vs.(2.29±0.60)mL]and wound bleeding requiring suture rate[3.3%(3/92)vs.5.6%(5/90)](all P>0.05).Conclusions The gauzer fixation positioning method has the advantages of accuracy of frenulum alignment and appearance satisfaction in circumcision with suture device,without increasing intraoperative blood loss and suture rate for wound bleeding.The method is simple and has good clinical effect,which is worthy of clinical promotion.
