摘　　要：目的:分析湖北省基层医疗机构门诊处方用药情况及影响因素,为提高合理用药水平提供参考。方法:选取湖北省四市17家基层医疗机构,采用WHO/INRUD医疗机构合理用药指标体系及其评价标准,分析门诊处方用药情况,利用多元线性回归及二元Logistic回归模型分析其影响因素。结果:基层医疗机构存在注射剂和抗菌药物滥用问题,基本药物使用率未达到WHO推荐标准;地区、机构类型、年份、季节、性别、年龄、疾病类型会影响门诊处方用药行为。结论:湖北省不同地区基层医疗机构合理用药水平存在差异,需重点关注呼吸系统疾病合理用药问题。Objective To analyze the situation and influencing factors of rational drug use in outpatient prescriptions of primary medical institutions in Hubei province,so as to provide reference for improving the level of rational drug use.Methods Seventeen primary medical institutions in four cities of Hubei province were randomly selected.The indicators and evaluation criteria of WHO/INRUD index system for rational drug use were used to analyze the outpatient prescription drug use in primary medical institutions.Multiple linear regression and binary logistic regression models were used to analyze the influencing factors.Results There were problems of injection and antimicrobial drug abuse,and the utilization rate of essential drugs in the four cities did not meet the WHO recommended standards.The results of multivariate analysis showed that region,institution type,year,season,gender,age and disease type all affected outpatient prescription behavior.Dominance analysis showed that region and disease category were the main influencing factors of each index.Conclusion There are differences in the level of rational drug use among primary medical institutions in Hubei province,the rational drug use of respiratory diseases needs to be paid more attention.
